01/03 I got ready to go to Featherstone because Bradford are playing there this afternoon. We set off at 1300 and there at 1400. The traffic was very busy and my driver asked steward where's the disabled parking. He said it's full and indicated to park in the normal spaces. We parked our minibus and asked a steward where s the wheelchair entrance for away supporters. He pointed to more Bradford supporters in wheelchairs and told us to wait with them. Another steward took us at the side of the stand where there was an old elevated viewing platform. It was not under cover, full of tables and plastic chairs with no seats for our carers to sit on. We found out that away wheelchair supporters should go to a new stand with a covered elevated viewing platform which was empty. Another 2 Bradford wheelchair supporters joined and weather got worse. In the end I told my carer I want to go home at 1445. We went back to our minibus and got home at 1545. I put my feet up and watched TV.
02/03 In the morning I used my PC to change my password plus I emailed Sarah Williams at the and in the afternoon I put my feet up and watched TV.
03/03 My nurse changed Catheter this morning. After I used my PC and replied to Sarah Williams to send my email to Featherstone. After lunch I put my feet up and watched TV. At 1600 my bowels opened and my carers changed my pad plus my shirt. After tea I went on my PC.
04/03 I got dressed up because I'm going to Maxis Chinese Restaurant n Leeds and the table is booked for 1900. Unfortunately at 1600 I had a terrible stomach ache and it lasted all evening. I told the nurse and 1730 I texted my sons to tell them I won't be going. I put my feet up and watched TV. I realised that I had Constipation because my bowels hadn't been open for at least 2 days.
05/03 In morning I used my PC. At 1130 I attended my monthly Care Plan review which lasted until 1230. In the afternoon I put my feet up and watched TV.
06/03 I was suppose to go to Crossgates in the Access bus but I was constipated and felt ill. I stayed in bed all morning. I got up in the afternoon, put my feet up and watched TV.
07/03 I worked on my PC in the morning. After brunch I put my feet up and watched TV.
08/03 Got ready to go to Odsal to see Bradford v Hunslet. We are setting off at 1130. We arrived at 1315 due to Satnav. We went into the Sponsors stand to have our 2 course meal which was Chicken stew, dumpling and chocolate cake. After we went into the viewing gallery to watch the match. At half time coffee and biscuits were served At full time we left missing man of the match. We got at 1800, I put my feet up and watched TV.
09/03 I had to get up at 0700 but the night forgot! I woke up at 0815 and told the carers I had an appointment at the dentist at 1125. I got a bed bath, cleaned my teeth and dressed. After breakfast my transport came and we got to the dentist at 1030. At 1120 I went in his surgery where he trimmed the mould for my new top set of false teeth. This finished at 1200 and we went downstairs to wait for our transport. It arrived at 1345 and we got home via LGI at 1430. I had my lunch, went to my room and used my PC.
10/03 In the morning I used my PC. I had lunch at 1200 because I had an appointment at 1430 to get my Baclofin pump filled up. After lunch my bowels opened and my carers changed my pad. My transport came at 1330 and we arrived at Pinderfields at 1410. At 1420 I saw the consultant who filled my Baclofin pump up. The nurse told me the of my next appointment 14/08. We went to the waiting room but we had to wait until 1645 for transport. When got home I had tea and after I told the nurse the date of my next appointment, I have cancelled Access bus to Crossgates, asked if she's going to change my Care Plan about changing my Catheter bags and would she tell Activities that I want to go to Leeds/Bradford Airport followed by Fish and Chips at Murgatroyds in Yeadon. I went to my room and logged on to my PC.
11/03 I didn't get up until 1030 because nobody put my shower sling in the wash. After breakfast I used my PC. After lunch the nurse filed my finger nail. I went on my PC.
12/03 In the morning I used my PC. In the afternoon I went with Activities to my GP to pick up some prescriptions, to Tesco's shopping where I bought a mains extension and some Kit Kats. After tea I used my PC.
13/03 I went to the Coffee morning where we played Bingo and 2 Quizzes. After lunch I used my PC.
14/03 I got an email from First Direct Arena asking my requirements are when I go to see Paloma Faith next Saturday.21/03 at 1930. I told them that a wheelchair space and a seat next to me for my carer. I asked them what time does it finish ? Emailed Dewsbury Rams to ask if they have a package that includes a meal ? In the afternoon I put my feet up and watched TV.
15/03 My Air Condition Unit that I ordered on Weds 11/03 arrived on Thurs is waiting to be PAT tested with the handyman ! After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.
16/03 In the morning I used my PC. In the afternoon I used my PC, put my legs up and watched TV. After tea I used my PC.
17/03 In the morning my son, Andrew, came at 1130 but he had to leave because lunch was at 1230 because we're having afternoon tea at 1500. After lunch I used my PC. At 1500 I went to the Activity room for Coffee and Cakes where we had 2 quizzes.
19/03 I phoned AB Cars to confirm that they had a booking on Saturday at 1800 to me and carer to First Direct Arena and to return at 2300. I asked the carers if I could go to Tesco's and they said this afternoon. I used my PC. In the afternoon we went to Tesco at 1430, We got home at 1600, unpaked my shopping and I went on my PC.
20/03 This morning our Clerical asked me to transfer #70 and she will give #70 in cash. The handyman started to set my ACU up. I used my PC. In the afternoon I put my feet up ad watched TV. After tea I went on my PC.
21/03 This morning I got ready to see Paloma Faith at the First Direct Arena. I went on my PC. In the afternoon I put my feet up and watched TV. After tea I get ready because our taxi from AB Cars was coming at 1800. He arrived at 1755 and we got there at 1830. We took the lift to floor(2) where the wheelchair users sat at front. There was about 50 spaces with chairs next to spaces for carers. It was a good view. The first act came on at 2000 Vintage Trouble and played until 2030. Paloma Faith came on at 2100 and she finished at 2300. Our taxi was outside and got at 2330.
22/03 This morning I used my PC. At 1100 I felt sick and I put my feet up and watched TV. I didn't any lunch. I had tea in my room.
23/03 I bought a CD/Radio/Alarm Clock with a remote control. I phoned Bradford Bulls and ordered 1 wheelchair space, 1 carer, 1 adult and disabled parking space for the match at Dewsbury on Easter Monday.. I'm trying to do my Family Tree but I'm finding it difficult. After lunch my bowels opened and my carers changed my pad. I put my feet up and watched TV.
24/03 This morning I used my PC and I got my CD/Radio/Alarm Clock. In the afternoon I sent it to be PAT tested. I put my feet up and watched TV. After tea I used my PC.
25/03 This morning my eldest son came at 1130 and stayed until 1230 and after I used my PC. In the afternoon I put my feet up and watched TV.
27/03 Yesterday I stayed in bed until after lunch. I got up, put my feet up and watched TV. Today in the morning I used my PC. After lunch my GP to look at my legs that were red, scabby and swollen. She said it was Cellulitus and prescribe some anti-botics. She said I must keep my feet up.
28/03 In the morning I used my PC to order a Kidney bowl from Amazon. On Ebay I put 2 bids on for Proof Gold Sovereigns.
29/03 My bids on Ebay were out bid and in the afternoon I put my feet up plus I watched TV. At 1600 my bowels opened and my carers changed my pad. After I went on my PC working on my Family Tree.
30/03 I asked the carers if they would get some batteries for my remote controls. I find out that Activities were taking me to Dewsbury on Easter Monday. During the morning I put my feet up and used my PC. After lunch I put my feet up and watched TV. At 1600 my bowels opened and my carers changed my and polo shirt. They found another sling because mine was soiled. I told my nurse that since I on antibiotics my sling was soiled twice. She agreed and I'm going to see the physoxxxx when she returns. Also I'm going to see Activities to see if we can go to The Mermaid Fish restaurant after the match on Easter Monday. After tea I used my PC.
31/03 My GP came this morning to look at my Cellulites and he prescribed something to clear the wax in my ears. In the afternoon I phoned my wife because she wrote a cheque from my pension. For the rest of the afternoon I used my PC. After tea I used my PC.

My Life being Disabled - December
01/12 I phoned Leeds RL to buy a wheelchair space, a carer and disabled parking space for the match against Wakefield on Boxing Day which starts at 11:30. In the afternoon I, my driver and my OT went to Wheelchairs Services to pick up my new pink electric wheelchair. After tea I started changing my passwords.
02/12 I went to my Advocacy headquarters A4MHD in Cross Green because it was their Open Day. It started at 11:30 and we left at 13:30. On the way back we went B & M and Asda. We got home at 15:30.
03/12 I asked the nurse if I could go to The Championship Summer Bash held on 23/24 May in Blackpool. I finished changing my passwords. I got my tickets for The Boxing Day match between Leeds v Wakefield which starts at 11:30. In the afternoon I put my feet up and watched TV. At 15:30 my eldest came and me if I wanted to go to the German market in The Millennium Square. He said he'll ask the nurse.
04/12 I asked the nurse because she wasn't there when I went out. I used my PC before lunch. After lunch I put my feet up and watched TV. After tea I used my PC.
05/12 In the morning I went to the quarterly Care Plan review meeting where Ravensdale Nursing Home, my Advocate and myself discussed my Care Plan and it lasted 30 minutes.. In the afternoon I put my feet up and watched TV.
06/12 This morning I went to Tesco's for some shopping. When we came back I had Mulligatawny soup for lunch. In the afternoon I used my PC and watched TV.
07/12 I went to church in the morning. In the afternoon I used my PC and I asked the nurse if I could go to friendly between Bradford v Huddersfield at 15:00 on 18/01/15.
08/12 Overnight I restarted my PC. This morning I used my PC, activities came to tell we're booked After in at The Beehive tomorrow at 12:30 and confirmed they are taking me to Leeds city centre at 11:00. I txtd Andrew to let him know. After lunch I used my PC, put my legs up and watched TV.
09/12 I went to the Beehive in Thorner for Xmas lunch. I came home and used my PC.
10/12 I went to the German market in Leeds Millieum Square where I met Andrew, my eldest son, we had a German lunch. We went round the stalls and walked round the shops before we came home at 15:00. After I used my PC.
11/12 I used my PC in the morning. I filled in a questionnairre about Rugby League on the BBC site. I ordered a wheelchair user, a carer, a driver and disabled car space for Leeds v Bradford on 25/01/15 at 15:00. In the afternoon I put my feet up and watched TV.
12/12 In the morning I tidied my room and worked on my PC. In th afternoon I put my feet up and watched TV.
13/12 I filled a questionnaire in for Sky. In the afternoon I went to Tesco's for some shopping. We went in our minibus.
14/12 Last evening I went to bed early because I said I felt sick and this morning I missed church because the carers left me in bed until 11:00. After Sunday lunch I used my PC and I put my feet up and watched TV.
15/12 I got up at 07:00 because I had an appointment at 10:40 with the Orthotics at Pinderfields to try my new shoes on. My transport arrived at 09:00 and arrived at Pinderfields at 09:40. We called in at 10:30 and were finished at 10:40. We told the receptionist we were ready for transport but it arrived 12:30. We boarded but they didn't have extension leads for my safety belt but the driver moved me to accommodate this. We got home at 13:15. After lunch I put my feet up and watched TV.
16/12 I used my PC and started to tidy my room. I phoned Leeds RL to ask for a Disabled Parking space but they told me to email stephiewright@leedsrugby.com who deals with the requests. I tried but I didn't get through. After lunch I put my feet up and watched TV.
18/12 This morning I got up at 07:00 because I had an appointment at St James about my eyes at 11:00. My transport arrived at 09:00 and we arrived at St James at 09:40. We were called in at 10:50 and my were tested. I was informed that my eyesight was good enough to drive my electric wheelchair as Wheelchair Services had asked. We told the receptionist to book our transport home at 11:00. The transport came at 13:00 and we arrived home at 13:30. After lunch I put my feet up and watched TV.
19/12 In the morning I went to a Coffee morning where I won a mug playing Bingo. In the afternoon I put my feet up and watched TV. I set my PC to reset because it running slow.
20/12 In the morning I used my PC. In the afternoon I put my feet up and watch TV.
21/12 I had asked to get up 06:30 but my sling was wet so I got up at 07:30 to go to church. The access bus came at 09:20 and we arrived at 09:40. The service lasted until 11:30 and we got home at 11:50. In the afternoon my sister an her husband came. They brought me a Hyacinth. After I used my PC.
22/12 In the morning I used my PC and got a Xmas card from my MP. I also got a letter from Leeds MDC about my name and my address to which I replied. This afternoon I went to Tesco's with 2 other residents. We set off at 14:00 and returned at 16:30.
24/12 I got a letter this morning about Bowel cancer, asking me had I done the test. I had pass it on to the nurses and the centre hadn't received it. I gave it to the manager to deal with. Later the nurse came to tell me if my bowels open I should tell my carers the nurse has to examine it. After lunch I phoned the HC1/2 helpline to order a new HC1/2 form. I put my feet and watched TV.
25/12 My sons came with their girlfriends n Xmas day morning. We had Tomato soup, Turkey etc. and Xmas pudding for lunch. After lunch I put my feet up and watched TV.
26/12 I went to watch Leeds v Wakefield which started at 11:30. We got home at 14:30 and I put my feet up and watched TV.
27/12 In the morning I used my PC. After lunch I put my feet up and watched TV.
28/12 I went to St James Church in the morning. In the afternoon I put my feet up and watched TV.
29/12 I went to Leeds General Infirmary to get some new hearing aids. My appointment was 09:15 and we came back at 11:45. I used my PC to pay my 4 weekly bill to Ravensdale Health Care Ltd. In the afternoon I put my feet up and watched TV.
30/12 I used my PC all morning. In the afternoon I put my feet up and watched my TV.
31/12 My buzzer lead got caught in my wheelchair and when the carers had released it they charged my battery. In the afternoon I put my feet up and watched TV.

05/02 I had my Care Plan monthly review in the morning where we agreed on these matches, Leigh away 15/02 1500, Featherstone away 01/03 1500, Hunslet home 08/03 1500, Dewsbury away 06/04 1500, Doncaster away 26/04 1400. After lunch I phoned the Access bus to tell them to cancel my Sunday visit to the church and make my visit to Crossgates a 2 weekly visit. I got an appointment to see Dr Kent on 13/02 at 1000. After I put my feet up and watched TV.
06/02 I went to Crossgates in the Access bus shopping this morning. In the afternoon I used my PC.
07/02 Trying to sort out a taxi on 21/03 1930 at the first direct area. I booked one from A B Taxis for 1800 return 2300. After lunch I put my feet up and watched TV. After tea I used my PC.
08/02 I used my PC and got 2 extension leads from Amazon in the morning. I checked my fixture list to find the next match was Featherstone away 1500 after Leigh away. In the afternoon I put my feet up and watched TV. After tea I used my PC.

10/02 I tidied my room in the morning. After lunch I used my PC. I went to the Halton Clinic to see a Podiatrist to get my toenails cut.
11/02 I had an appointment at Gaforth Clinic to physio at 1000. The Patient transport came at 0915 and we arrived at 0945 but the receptionist told us I didn't have an appointment. We got home at 1100. My eldest son came at 1200 and left at 1300. After lunch I used my PC, put my feet and watch TV. After tea I used my PC.
12/02 I sent email to the Practice Manager for my GP complaining about yesterday. I emailed Bradford Bulls to find out how much is cost to get in the Coral stand. In the afternoon I put my feet up and watched TV.
13/03 I went to see my Neurologist to have my 6 monthly check. After she said we'll change it to yearly. After lunch I used my PC. Activities held a quiz for the Service User.
14/02 This morning I used my PC and I bought a Sovereign from The Gold Bullion Co. I sent email to PALS asking to get a copy of all health records. In the afternoon I watched Catalans beat Castleford 13 - 12.
15/02 I went to Leigh to watch Bradford. We set off at 1215 and arrived 1315. We went into Morrisons and had a Big Breakfast. After we went in the ground but should gone upstairs viewing platform but the stewards to us to pitch side wheelchair users. Leigh won 36 - 24. When we got home I ordered a Nawebi Khana with Garlic Naan from Jinnah.
16/02 I went to my dentist in Chapellerton this morning. After lunch I watched a movie and tea used my PC.
17/02 I sent Sarah Williams my report from Leigh. I tried to get tickets for the Featherstone away but Bradford didn't have any and Featherstone said they have sent Bradford their allocation. I asked the nurse if I could go Maxi's at 1900 on 04/15. At 1330 the Wheelchair repair came and temporarily fixed my wheelchair. I used my PC for the rest of the afternoon.
18/02 I got my 2015 Gold Sovereign this morning. Later I got my flat cap from Amazon. I asked the carers if I could go to Tesco's.
19/02 We had a quiz in the morning and I won. In the afternoon I phoned Bradford Bulls and asked them if they were selling tickets for the Featherstone match - No. In the afternoon I used my PC.
20/02 In the morning I went to Crossgates shopping. After lunch I used my PC but I couldn't access Santander. I phoned them to find out I never changed my address. I watched a movie for the rest of the afternoon.

24/02 The Wheelchair repairs are coming this afternoon. I was suppose to have a bladder wash but the nurse told me she didn't think it was necessary. She's going to speak with my GP. The Wheelchair repair man came and fix my wheelchair. I had a bladder wash at 23:00 when I went to bed.
25/02 I got a letter from my GP's Practice manager about a mix up when I went to the Physio but I should got a home visit. In the afternoon I put my feet and watched TV.
26/02 I used my PC all morning. In the afternoon I put my feet u and watched TV.
27/02 I went to the Coffee morning in the Activities room where the cook talked to the residents about the new menu. After we listened to music. After lunch I went on my PC and I put my feet up while watching TV.

05/02 I had my Care Plan monthly review in the morning where we agreed on these matches, Leigh away 15/02 1500, Featherstone away 01/03 1500, Hunslet home 08/03 1500, Dewsbury away 06/04 1500, Doncaster away 26/04 1400. After lunch I phoned the Access bus to tell them to cancel my Sunday visit to the church and make my visit to Crossgates a 2 weekly visit. I got an appointment to see Dr Kent on 13/02 at 1000. After I put my feet up and watched TV.
06/02 I went to Crossgates in the Access bus shopping this morning. In the afternoon I used my PC.
07/02 Trying to sort out a taxi on 21/03 1930 at the first direct area. I booked one from A B Taxis for 1800 return 2300. After lunch I put my feet up and watched TV. After tea I used my PC.
08/02 I used my PC and got 2 extension leads from Amazon in the morning. I checked my fixture list to find the next match was Featherstone away 1500 after Leigh away. In the afternoon I put my feet up and watched TV. After tea I used my PC.
09/02 I got up at 0700 because I had an appointment with the dentist but at 0930 they phoned to tell me it was cancelled. In the afternoon I went to Seacroft Tesco's and on the way back I came off footpath again. After I emailed Leeds MDC again.
10/02 I tidied my room in the morning. After lunch I used my PC. I went to the Halton Clinic to see a Podiatrist to get my toenails cut.
11/02 I had an appointment at Gaforth Clinic to physio at 1000. The Patient transport came at 0915 and we arrived at 0945 but the receptionist told us I didn't have an appointment. We got home at 1100. My eldest son came at 1200 and left at 1300. After lunch I used my PC, put my feet and watch TV. After tea I used my PC.
12/02 I sent email to the Practice Manager for my GP complaining about yesterday. I emailed Bradford Bulls to find out how much is cost to get in the Coral stand. In the afternoon I put my feet up and watched TV.
13/03 I went to see my Neurologist to have my 6 monthly check. After she said we'll change it to yearly. After lunch I used my PC. Activities held a quiz for the Service User.
14/02 This morning I used my PC and I bought a Sovereign from The Gold Bullion Co. I sent email to PALS asking to get a copy of all health records. In the afternoon I watched Catalans beat Castleford 13 - 12.
15/02 I went to Leigh to watch Bradford. We set off at 1215 and arrived 1315. We went into Morrisons and had a Big Breakfast. After we went in the ground but should gone upstairs viewing platform but the stewards to us to pitch side wheelchair users. Leigh won 36 - 24. When we got home I ordered a Nawebi Khana with Garlic Naan from Jinnah.
16/02 I went to my dentist in Chapellerton this morning. After lunch I watched a movie and tea used my PC.
17/02 I sent Sarah Williams my report from Leigh. I tried to get tickets for the Featherstone away but Bradford didn't have any and Featherstone said they have sent Bradford their allocation. I asked the nurse if I could go Maxi's at 1900 on 04/15. At 1330 the Wheelchair repair came and temporarily fixed my wheelchair. I used my PC for the rest of the afternoon.
18/02 I got my 2015 Gold Sovereign this morning. Later I got my flat cap from Amazon. I asked the carers if I could go to Tesco's.
19/02 We had a quiz in the morning and I won. In the afternoon I phoned Bradford Bulls and asked them if they were selling tickets for the Featherstone match - No. In the afternoon I used my PC.
20/02 In the morning I went to Crossgates shopping. After lunch I used my PC but I couldn't access Santander. I phoned them to find out I never changed my address. I watched a movie for the rest of the afternoon.
21/02 In the morning I used my PC. In the afternoon I put my feet and watched TV.
22/02 I used my PC in the morning. I put my feet up and watched TV in the afternoon.
23/02 I went to the dentist this morning. He filled a tooth and took an impression of my uppers. He reminded me that my next appointment was 09/03 at 1100. After we waited 3 hours for our transport. We went home and got home at 1545.
24/02 The Wheelchair repairs are coming this afternoon. I was suppose to have a bladder wash but the nurse told me she didn't think it was necessary. She's going to speak with my GP. The Wheelchair repair man came and fix my wheelchair. I had a bladder wash at 23:00 when I went to bed.
25/02 I got a letter from my GP's Practice manager about a mix up when I went to the Physio but I should got a home visit. In the afternoon I put my feet and watched TV.
26/02 I used my PC all morning. In the afternoon I put my feet u and watched TV.
27/02 I went to the Coffee morning in the Activities room where the cook talked to the residents about the new menu. After we listened to music. After lunch I went on my PC and I put my feet up while watching TV.

My Life being Disabled - Jan 15
01/01 Changing my passwords today but I'm having difficulty.
02/01 Went to Crossgates shopping centre via the Access bus in the morning. I:00n the afternoon I put my feet up and watched TV. In the evening about 18:00 I felt funny, I asked the carers could go to bed at 21:00. At 21:30 I told the nurse that I felt better and she told my GP. I feel asleep and later.
03/01 Tried to connected my PC to my TV but failed. I used my PC all morning. In the afternoon I connected my PC with my TV. I put my feet up and watched TV.
04/01 I went to St. James church in the morning using the Access bus. After lunch the nurse tidied my room. After I put my feet up and watched TV.
05/01 A carer cut my hair and I asked another to book an appointment for the podiatry ASAP.
06/01 In the morning I used my PC and in the afternoon I put my feet up and watched TV.
07/01 After breakfast I was taken to St James A and E because my right hand was severely bruised. They took several X-rays and the doctor said it wasn't broken, They bandaged my right elbow and put my right in a sling. We came at 14:30, I had a Tuna sandwich and went to my room. I tidied it and logged on to my PC.
08/01 I emailed the manager of Ravensdale to complain the my washing is getting shrunk and 6 my hankies are pink. I organised a demo on a Citroen Berlingo Multispace with Automotive Group National for 11:00 on 26/01/15.
09/01 I got up at 07:00 but when I was being hoisted my bruised right started hurting. The carers put me back into bed and I asked the carers to phone Access bus to tell them I wouldn't be going to Crossgates. I got up at 10:00 and I had breakfast. I tidied my room and used my PC. I got the footstool from Amazon. In the afternoon I put my feet up and watched TV.
10/01 I used my PC in the morning. I ordered a Duronic HDM1 splitter, an aerial lead and 4 HDM1 leads from Amazon. I put my feet up and watched TV in the afternoon. After tea I went on my PC.
11/01 I got up at 07:20 because the carers were busy. I didn't have any breakfast and only half a cup of coffee. We to St James church for Holy Communion and we got home I put my sling on. I went on my PC and I asked if I could to watch Shaw Cross v York Acorn on 31/01/15 at 14:00.
12/01 In the morning I used my PC. In the afternoon I used my PC until 16:00 when I went down to the other unit where we watched a woman sing songs that were popular during World War 2. It lasted about 1 hour. I had Salmon fillet for tea.
13/01 In the post I got my disabled parking pass for the match Leeds v Bradford 25/01 at 15:00 which is Jamie Peacocks Testimonial. I used my PC in the morning. In the afternoon I used my PC, put my feet up and watched TV.
14/01 I got a HDM1 switcher and a TV Aerial lead from Amazon this morning. I emailed the manager of Ravensdale about a new service user who keeps coming into my room, I asked him when somebody is going to check my possession, can I pay Ravensdale monthly instead 4 weekly and do you have any procedures that cover a service user getting a Mobility WAV. In the afternoon I used my PC and I went to Thorpe Park Clinic near Colton to get my Hearing Aids repaired. From 18:15 to 23:00 the new service user kept trying to get in my room.
15/01 I was woken by him at 06:00 trying my door again. I used my PC and the new service user kept coming in my room. After lunch my wife phoned. I used my PC all afternoon. My prescription for my dressing arrived for my arm and the nurse fax it to Boots.
16/01 I went to Crossgates shopping in the morning. After lunch I used my PC.
17/01 Last night the Service User kept banging on my door until 22;00. This morning he keeps coming into my room. I reported this to Social Services Direct. After lunch I used my PC and the cook asked if I wanted my Pizza with Bolognese sauce. I used my PC still being disturbed by Service User. After tea I used my PC.
18/01 I went to church this morning and after lunch I used my PC.
19/01 I emailed the secretary of the Leeds East Killingbeck and Seacroft branch of thr Labour Party to find when have meeting, where they are and is wheelchair accessible. He emailed me back and the next meeting is tomorrow starting at 1900. In the afternoon I used my PC.
20/01 I used my PC and order a home shirt plus an away shirt from Bradford Bull shop. In the afternoon I spoke to Sky to get my Sky TV, phone and broadband moved from room 19 to room 10 on 23/01.
21/01 My phone was disconnected this morning. After tea I used Activities connection.
22/01 After tea I used Activities connexion. I booked a table for 6 at Maxis Chinese restaurant for next Weds at 1830.
23/01 Sky came to move Sky TV to room 10. I went to Crossgates shopping and when I came home all my possessions had been moved into room 10.
24/01 I got a text from the car dealer to ask me that I will give him prove of DLA Benefit and driving licences of 2 drivers. I went to the Activities room to use their Broadband. In the afternoon I put my feet and watched TV. After tea I used my PC until 1800.
25/01 This morning I went to St. James church in Seacroft and after lunch I went to watch Leeds v Bradford 34 - 14. When I got home I ordered a Chicken Nabawi Khana from Jinnahs York Road.
26/01 I met the rep from mobility with a Citreon Berlingo but it too small he told me they have bigger one which costs 500 pounds more and we agreed to meet on 04/03 at 11:00. After lunch I put my feet and watched TV. After tea I used my PC.

28/01 I used my PC in the dining room and after lunch I put my feet up and watched TV.
29/01 I used my PC in the quiet lounge.
30/01 I didn't go to Crossgates with the Access bus because I couldn't get up the ramp. The nurse called my GP because my Cellulitus was flaring up. He came in the afternoon and presciped some anti-bioptics.

My Life being Disabled November
01/11 Used my PC in the morning, I watched a New Zealand v Samoa from New Zealand with New Zealand winning 14 - 12. I had Sirloin steak for tea and the cook made me garlic sauce.
02/11 Got up at 07:00 to go St James church near Tescos. The service started at 10:00 and after we went to Tescos. After Sunday lunch I went on my PC. After I put my feet up and watched TV.
03/11 I restored my PC because it had been corrupted. In the afternoon my bowels opened and I put my feet up to watched TV. After tea my bowels opened again and my carers changed my pad plus my joggers.
04/11 Going to Asda after lunch my bowels opened and my carers changed my pad. I tried to get my printer working.
05/11 Went to Seacroft hospital/Wheelchair Services to try my new wheelchair but I couldn't bring it home because the manufacturer had changed their specifications and didn't tell Wheelchair Services. After lunch I tried my printer again and got it going. I printed my return from Amazon x 3.
06/11 In the morning I used my PC and in the afternoon I watched TV.
08/11 In the morning I used my PC and in the afternoon I watched New Zealand beat England 16 - 14. Before I thought my bowels had opened but it was a false alarm.
09/11 I went to St James church for Remembrance Sunday. After we went to Tesco's to buy things. After Sunday lunch I watched the TV and put my feet up.
10/11 The OT told me she has referred me to my GP to get some physio. Larer I used my PC.
11/11 I put my Samsung Galaxy Young mobile on Ebay. In the afternoon my bowels opened, I put my feet up and watched TV.
12/11 I asked the nurse if she had organised a carer to go with me to Jinnahs tonight to meet Andrew my eldest son but she hadn't. She said she will organise one. She told me my 1.5l catheter bags are no longer supplied by Colorplast. At 18:30 we went to Jinnahs to meet my son. A carer stayed with me and I had Meat Samosas plus Chicken Nawabi Khana. The meal and the service good but I had to cope with a step to get in which was unpleasant !
13/11 My GP came this morning because my right leg was red and sore. He said it was Cellulitis and he gave me a prescription for antibiotics. I received 2 letter 1 from St James for an appointment on the 18/12 at 11:30 and 1 from the Tax office. I gave my appointment to the nurse and she must get my GP to request transport. In the afternoon I put my feet up and watched my TV. After I used my PC.
14/11 I went to the Coffee morning before lunch. After I restored my PC because it wouldn't go on the internet. Later I put my feet up and watched TV.
15/11 In the morning I tidied my room and used my PC. In the afternoon I watched the final of the 4 Nations with New Zealand beating Australia 22 - 16. After tea I used my PC.
16/11 During the night I woke with stomach ache. I couldn't get back to sleep. I stayed in bed until 11:00. After lunch I used my PC.
17/11 In morning I had appointment to see a Poidintry to get the toenails at Seacroft Clinic. In the afternoon I put my feet and watch TV. After tea I had headache and right/paralysed was tingling. At 20:00 the nurse called a doctor but he didn't arrive until 23:30. By that time I'd taken my night medicine and I was barely awake. He checked my vitals and left. I slept all night.
18/11 I was alright in the morning until my cleaner asked if she could clean my carpet. I went to the dining room at noon to read my letters. Again my pains occurred but at 13.00 with the noise I went room and my carers put me in my bed. At 14:00 the nurse phoned my GP and told her told her to phone for an ambulance. It arrived at 14:45 and we arrived at St James at 14:45. I went the A&E after seeing the doctor and my chest X-Ray I was diagnosed a Chest problem. I was anti-biotics and told I was admitted at 18:45 once they find a bed. I told them I use an Air Filled mattress and they said they would order one. From 18:45 until 23:45 I stayed in A&E. I was moved to J26 but kept near the Nurse Station. One of the notices said ' Please tell the NUSES ' ? I got a bed at 01:30 and went sleep at 02xx after I was administered Pracetamol and a Sleeping pill.:
19/11 I woke up at 07:00, I had Porridge and toast for breakfast. I told the catering staff that I had El-Hal meat. I arranged with Hospedia to have television, phone plus internet cost of 25 pounds ! I tried to contact my banks through the internet but it was too slow. I tried to get them via the phone but 0844 numbers are banned ? I phoned Service Desk to ask why they didn't know. I told them my internet was very slow. An engineer came and speeded up. I had a curry, naan bread and a dessert for lunch. At 16:00 a nurse came and told I was being discharged. At 19:00 I asked Hospedia to me of their system. About 11:00 I asked the nurse if I could go to sleep. She dispensed my night time pills and I went to sleep/ At about 01:00 someone woke me to tell me my transport was on it's way. She packed my bags and got me ready. At 01:15 my transport arrived I got home at 02:00.
20/11 I woke up at 09:30 after breakfast I used my PC. In the afternoon I put feet and watched TV.
21/11 I went to the Coffee morning where we had quizzes. In the afternoon I went Tesco's.
22/11 I used my PC in morning. I sent an email to Hospedia. After lunch I put my feet up and watched a movie. After tea I used my PC.
23/11 I had to evacuate my room because the domestics were cleaning my carpet. After breakfast in the TV room because the painters were painting the dining room I went to the Activities room. I took my PC with me to tidy it up. I asked if they have heard from the council because I havn't received my disabled bus. They were chasing it. They told me that they plan going to The Beehive, Thorner on 02/12 and they will arrange it. I got an appointment on 29/12 09:15 at the LGI to get my new hearing aids. I booked my transport and gave the to the nurse. After lunch I went to my room and used the internet.
24/11 I had an appointment at The Reginald Centre in Chapeltown to see a dentist. The Patient Transport arrive at 10:00 and we arrived 10:30. We guided to the dentist's surgery upstairs and 11:10 we were called. He examined my teeth. He said I want some fillings and cleaning. He said do I want a new set of my upper false teeth. I said they were loose and I wanted a new set. He said I have 6 appointments. We went to the receptionist and she put 6 appointments in their diary, giving me a printed copy. I asked her would she arrange transport. We went down stairs to wait for the Patient Transport at 11:40. At 12:40 I phoned their control who told me they havn't allocated a vehicle YET. My carer phoned my home and asked if our minibus was free YES. The driver arrived at 13:00 just as Patient Transport arrived. We informed the driver saying your control told me at 12:40 there was nobody allocated. We got at 13:20 but Steve our handiman took me to B&Q where I bought 2 glass shelves for my bathroom. On the way home I bought a Haddock special and mushy peas.
26/11 This morning I asked the nurse to call my GP because I had a Hernia in my chest. My advocacy came to invite me to their Xmas party. She came 13:30 and said don't bother because it's my stomach. She raised my stomach pills I asked her to sign my disabled bus (pass application form. I told her my amaphilin(3) wasn't working. She raised to 4 pills. At 14:30 Activities were having a Rock Horror Show. It lasted until 17:00 where I changed of my visit to The Beehive and asked if they would me A4MHD for their Open Day.
27/11 I used my PC before lunch. I was going to Leeds RL and Aldi but the handyman put my glass shelves up.
28/11 I phoned Leeds RL and for person who received my scan but they were out. The activities came to ask me if I wanted to go to Crossgates. The said be ready at 11:30. In Crossgates I bought 15 boxes of Chocolate Liqueers, Xmas Tree and baubles that light up at Wilkos. In Tesco I bought 4 boxes of Pringles, a Sirloin steak and some mustard. We got home at 14:30 and had lunch. In the afternoon I used my PC.

I received 2 letter 1 from St James for an appointment on the 18/12 at 11:30 and 1 from the Tax office. I gave my appointment to the nurse and she must get my GP to request transport. In the afternoon I put my feet up and watched my TV. After I used my PC.
14/11 I went to the Coffee morning before lunch. After I restored my PC because it wouldn't go on the internet. Later I put my feet up and watched TV.
15/11 In the morning I tidied my room and used my PC. In the afternoon I watched the final of the 4 Nations with New Zealand beating Australia 22 - 16. After tea I used my PC.
16/11 During the night I woke with stomach ache. I couldn't get back to sleep. I stayed in bed until 11:00. After lunch I used my PC.
17/11 In morning I had appointment to see a Poidintry to get the toenails at Seacroft Clinic. In the afternoon I put my feet and watch TV. After tea I had headache and right/paralysed was tingling. At 20:00 the nurse called a doctor but he didn't arrive until 23:30. By that time I'd taken my night medicine and I was barely awake. He checked my vitals and left. I slept all night.
18/11 I was alright in the morning until my cleaner asked if she could clean my carpet. I went to the dining room at noon to read my letters. Again my pains occurred but at 13.00 with the noise I went room and my carers put me in my bed. At 14:00 the nurse phoned my GP and told her told her to phone for an ambulance. It arrived at 14:45 and we arrived at St James at 14:45. I went the A&E after seeing the doctor and my chest X-Ray I was diagnosed a Chest problem. I was anti-biotics and told I was admitted at 18:45 once they find a bed. I told them I use an Air Filled mattress and they said they would order one. From 18:45 until 23:45 I stayed in A&E. I was moved to J26 but kept near the Nurse Station. One of the notices said ' Please tell the NUSES ' ? I got a bed at 01:30 and went sleep at 02xx after I was administered Pracetamol and a Sleeping pill.:
19/11 I woke up at 07:00, I had Porridge and toast for breakfast. I told the catering staff that I had El-Hal meat. I arranged with Hospedia to have television, phone plus internet cost of 25 pounds ! I tried to contact my banks through the internet but it was too slow. I tried to get them via the phone but 0844 numbers are banned ? I phoned Service Desk to ask why they didn't know. I told them my internet was very slow. An engineer came and speeded up. I had a curry, naan bread and a dessert for lunch. At 16:00 a nurse came and told I was being discharged. At 19:00 I asked Hospedia to me of their system. About 11:00 I asked the nurse if I could go to sleep. She dispensed my night time pills and I went to sleep/ At about 01:00 someone woke me to tell me my transport was on it's way. She packed my bags and got me ready. At 01:15 my transport arrived I got home at 02:00.
20/11 I woke up at 09:30 after breakfast I used my PC. In the afternoon I put feet and watched TV.
21/11 I went to the Coffee morning where we had quizzes. In the afternoon I went Tesco's.
22/11 I used my PC in morning. I sent an email to Hospedia. After lunch I put my feet up and watched a movie. After tea I used my PC.
23/11 I had to evacuate my room because the domestics were cleaning my carpet. After breakfast in the TV room because the painters were painting the dining room I went to the Activities room. I took my PC with me to tidy it up. I asked if they have heard from the council because I havn't received my disabled bus. They were chasing it. They told me that they plan going to The Beehive, Thorner on 02/12 and they will arrange it. I got an appointment on 29/12 09:15 at the LGI to get my new hearing aids. I booked my transport and gave the to the nurse. After lunch I went to my room and used the internet.
24/11 I had an appointment at The Reginald Centre in Chapeltown to see a dentist. The Patient Transport arrive at 10:00 and we arrived 10:30. We guided to the dentist's surgery upstairs and 11:10 we were called. He examined my teeth. He said I want some fillings and cleaning. He said do I want a new set of my upper false teeth. I said they were loose and I wanted a new set. He said I have 6 appointments. We went to the receptionist and she put 6 appointments in their diary, giving me a printed copy. I asked her would she arrange transport. We went down stairs to wait for the Patient Transport at 11:40. At 12:40 I phoned their control who told me they havn't allocated a vehicle YET. My carer phoned my home and asked if our minibus was free YES. The driver arrived at 13:00 just as Patient Transport arrived. We informed the driver saying your control told me at 12:40 there was nobody allocated. We got at 13:20 but Steve our handiman took me to B&Q where I bought 2 glass shelves for my bathroom. On the way home I bought a Haddock special and mushy peas.
26/11 This morning I asked the nurse to call my GP because I had a Hernia in my chest. My advocacy came to invite me to their Xmas party. She came 13:30 and said don't bother because it's my stomach. She raised my stomach pills I asked her to sign my disabled bus (pass application form. I told her my amaphilin(3) wasn't working. She raised to 4 pills. At 14:30 Activities were having a Rock Horror Show. It lasted until 17:00 where I changed of my visit to The Beehive and asked if they would me A4MHD for their Open Day.
27/11 I used my PC before lunch. I was going to Leeds RL and Aldi but the handyman put my glass shelves up.
28/11 I phoned Leeds RL and for person who received my scan but they were out. The activities came to ask me if I wanted to go to Crossgates. The said be ready at 11:30. In Crossgates I bought 15 boxes of Chocolate Liqueers, Xmas Tree and baubles that light up at Wilkos. In Tesco I bought 4 boxes of Pringles, a Sirloin steak and some mustard. We got home at 14:30 and had lunch. In the afternoon I used my PC.
29/11 I used my PC in the morning Afternoon I watched England beat Australia
30/11 I used the Access Bus to take me and my carer to St James for Holy Communion and to take us back. This afternoon I used my PC.

My Life being Disabled October
- 01/10 I LOST ALL MY BLOGS FOR AUGUST AND SEPTEMBER In the afternoon put my feet up and watched TV. My eldest son came and brought my mobile - repaired.
- 02/10 I had a bladder wash and my blood/sugar was 9.5. After lunch my bowels opened and carers changed my pad. Somebody from Activities asked what I wanted from my birthday I told her I wanted a new pocket for my wheelchair. At tea time I felt sick and went to my bedroom.
- 03/10 I was weighed at 88.7kgs. I phoned my wife to inform her that my spending money hasn't been paid in.
- 05/10 I'm having problems with my laptop.
- 06/10 Trying to change my passwords. In the afternoon we went to Poundland and Tesco.
- 07/10 My 60th Brithday presents and cards. We went to Capri Bistro at 19:00 and met my two sons and their girlfriends.
- 08/10 I met somebody who told me, she had informed the manager and she wanted interview me. She asked me a few questions and asked me if I would like my benefits paid into my own account. I phoned the RFL to find out why my tickets havn't arrived. The ticket were sent to Dearnevale but the ticket office will have a duplicate copy on Saturday. I ordered a Lenovo G50-30 from Amazon.
- 12/10 We(myself,a driver and 2 carers) went to Old Trafford yesterday to watched Rugby League Grand Final between St. Helens and Wigan we set at 13:30 and got there at 16:00. I had to pay £9 for a parking space and we had to go to the ticket office because my tickets didn't arrived. They me a duplicate copies and we went through the wheelchair entrance. We went to the Ability Lounge where I got a steak pie, a packet of crisps and cup of coffee. (a special deal). We went to our places in front of wheelchair spaces behind the posts. There was entertainment until the match started and after 2 minutes a Wigan player got sent off for punching somebody. At half-time Wigan were winning at 6 - 4. The final score was Wigan 14 - 6. After the presentations it took 45 minutes to get out of the car park. We got home at 22:30.
- Mon 13/10 I went to the Pedaitory to get my toenails cut. My appoitment was for 08:40 but didn't arrive until 09:00 because our minibus was in use. We caught a bus to Seacroft and walked to The Grange Medical Centre but we arrived at 09:00 and was told we had missed my appointment. We went to Tescos shopping and caught a bus back home.
- Tues 14/10 Nicholas Flether from Leeds City Council came to investigate 2 problems I had flagged up to him. I started using my new PC all afternoon.
- Weds 15/10 I tried to get my new PC going but in the afternoon I went to the LGI to have my ears tested. My appointed was at 16:15 but I went in early and finished 16:45. Our transport came at 17:40. We got at 18:30 and I oRdered a Bolgnese pizza.
- Thurs 16/10 Myself, another resident, our driver and 2 carers went to White Rose shopping centre. I bought 2 polo shirts from BHS and I went with the driver to Frankie & Bennys for lunch. We got home at 15:30.
- Fri 17/10 My GP came because my blood/sugar was high, about 20 when it should 5 -10. We decided to reduce my biscuit and crisps. I order a wheelchair bag from Amazon. I enquired about a Mobility Car, Dacia Logan MCV Ambiance 1.2 16.
- Sat 18/10 I used my new PC during the day and after tea I put my feet up and watched TV.
- Sun 19/10 I'd asked the night staff to get me up at 07:00 but they didn't get me until 07:30 which was too late to go to church. I feel asleep again and woke up at 09:00. My carers told me my mattress was deflated and changed it for a normal one. I used my PC all morning, I watched a movie in afternoon but before tea my bowels opened and my carers changed my pad.
- Mon 20/10 I went to The Thackray Museum this morning and got Fish 'n' Chips plus mushy peas for lunch. After lunch I used my PC.
- Tues 21/10 I set my alarm at 06:00 because the nurse told me at 06:30 was a busy time but carers came at 06:20. After doing my Cares I went to the dining room to have my breakfast at 07:45. At 08:05 a carer told me she was going to Pinderfields with me to see the Orthotics at 10:20. We arrived early at 09:40 but the person on Orthotics saw early. I gave him my shoes that been shrunk by the night staff who put then in the washing machine on a high setting. He asked me did I want the same style and colour. We were in his office 5 minutes and after we told the receptionist the we would transport to go home. The same vehicle came at 11:00 and we arrived at home 11:30. I spent most of the afternoon with my feet up watching TV.
- Weds 22/10 Wrote a letter to Mark Henry and PALS about my experience when the night staff put my white shoes in the washing machine. A carer filed my left finger nails. In the afternoon I put my feet up and watched TV. My eldest son came to visit me.
- Thurs 23/10 Dr Roberts is coming from Pinderfields at 13:30 to see me ? I asked the nurse for some shampoo and toothpaste. Dr Roberts never appeared and the nurse is going to find out tomorrow. For the rest of the day I used my Lenovo PC. I asked the nurse if I can go to Jinnah Indian restaurant.
- Fri 24/10 I went to a coffee morning in the Activities room. In the afternoon I went to Tescos.
- Sat 25/10 I tried to contact Leeds First Direct Arena but I didn't through. The nurse told me I could get tickets for Paloma Faith on 21/03/15. In the afternoon I put my feet up a nd watched TV. I got in touch with the First Direct Arena and ordered 1 wheelchair space and 1 carers seat in Block 102. After Sunday lunch I put my feet and watched TV.
- Mon 27/10 In the morning I used my PC. After lunch I carried on and I got some beard shears from Amazon which I didn't want. I ordered a printer from Amazon.
- Tues 28/10 Got up at 07:00 because my GP and nurse were coming at 09:40. They didn't arrive. I got the 2015 fixtures for Bradford Bulls and asked the manager if I could go to some of them. After lunch/brunch I was sick all over and my carers changed my shirt, jogging bottoms and wheelchair cover. I put my feet up and watched TV.
- Weds 29/10 This morning the cook told me that she's got some Salmon which I can have for tea. We walk to Tesco's and Poundland to shop. After lunch my bowels opened and my carers changed my pad and my jogging bottoms. I sent an email to Jinnah to ask if my wheelchair can in there restaurant but it failed.
- Thurs 30/10 This I made a DVD on my PC.
- Fri 31/10 In the morning we had a Halloween coffee morning and in the afternoon we had a scary movie in the Activities room. After tea I rebooted my PC because it was goiing slow.

My Life being Disabled July
I was moved to Ravensdale Nursing home, Naburn Walk, Whinmoor, Leeds LS14 2DA on Monday 07/07/14. I am just beginning to get round the move but this home has less residents(I5).
Sat 12/07 started to changed my passwords but having great difficulty. I ordered a disabled trolley from Amazon.
Sun 13/07 I went on my computer but got tied up with my Hotmail.
Mon 14/07 same prob but getting worse. I order a footstool from Amazon.
Tues 15/07 am I tidied my room. I bought a wallet, costers and hankies from Amazon.
Thurs 17/07 at 11:30 we went shopping in Crossgate near home. I had a sausage sarnies, custard and a coffee. We in Tesco and Wilkies where I bought a mirror. Later we went in the garden where we had 2 quizzes.
Fri 18/07 quiet day watching telly and sorting my room. I emailed to my MP George Mudie to intoduce myself
Sat 19/07 I got appointment to Dr Kent next Mon 28/07 at 10:00 at Pinderfields, I got my Wembley tickets but no Car parking. My youngest came and my came with him.
Fri 25/07 somebody Comms got my feed for broadband working room but BT are coming on Mon.
Sat 26/07 bought a WPB from Amazon.
Weds 30/07 booked a taxi to take us to Headingley on Fri from Wheels.
Fri 01/08 at 18:00 our(a carer with me) the taxi from Wheels took us Headingley we arrived at 18:45 and was charged £15. We went in but a steward told us to get in lift but we came back down because my place was in front Carnegie stand. It rained all time and being at the front we with every wheelchair use got soaked. It was a good match with Bradford winning 14 - 20. We waited for taxi and he came 22:10 and we got home at 23:30. I ordered a Chicken Jalfrezi with Garlic Naan bread from Mojos.
Sat 02/08 wrote my report to Sarah Williams at the RFL.
