01/06 In morning we went to the Freeport at Castleford. After lunch I made an appointment with an Orthopedic doctor to get a new splint for my right arm. I bought 3 tickets for Bradford v Dewsbury match on 14/06. I booked a taxi for Wednesday at 1900 to Francos restaurant to return at 2200. After lunch I used my PC, put my feet up and watched PC. After tea I used my PC.
02/06 I had an appointment at 1100 with my dentist. The Patient Transport didn't arrive until 1045 and after dropping another user at St. James' we got to the Reginald Centre at 1120. We checked in and my dentist called for me to check my new top set was OK. I said they were OK and we told his assistant to call for my transport at 1200. We went downstairs to use their PC but all the PCs wee in use. I joined the Leeds Libraries and I got on a PC in the Job Centre. Where I complained to Patient Transport at 1300. I tried to use my mobile but the chap who answered said he couldn't hear me. I sent an email to them and my transport came at 1400. We got home 1430 and I lunch. After lunch I went Tesco's and to the Post Office where I sent some shorts back to Amazon. I got at 1630 because it was very windy. After I used my PC and answered an email from Leeds City Council about the state of the footpaths in Whinmoor.
03/06 I invited Richard Burgon MP and Pauleen Graham from Leeds City Council to our event to support the Huntingdon's Disease Association on 20/06 at 1930. After I used my PC. After lunch I put my feet up, drafted my points for my Quarter Care Plan review and watched my TV. At 1600 my bowels opened and my carers changed my pad, sling and new shirt. After tea I used my PC.
04/06 I used my PC and wrote a shopping list for me to go to Tesco's this afternfoon. I sent an email to the taxi/private hire licensing dept. about my epics last night. In the afternoon I went to Seacroft Centre and came back at 1600. he carers told me my right elbow was bleeding. After the nurse dressed my elbow and I used my PC.
05/06 Yesterday I reported several cars blocking the footpath in Naburn Drive. The police came to see me and told them where it was. They were going to look in to it. At 1200 I had my quarterly Care Plan review and it lasted until 1245. After lunch I used my PC and later I put my feet up and watched TV. After tea I used my PC.
06/06 In the morning I had my weekly bladder wash before going to my bathroom. After breakfast I used my PC. After lunch I used my PC, put my feet up and watched my TV. After tea I used my PC.
07/06 After breakfast I started to get ready to go to Odsal to watch Bradford v Doncaster we're setting off at 1130 because we've go to be there at 1230. We set off and arrived at 1215 but the doors havn't opened and we wait until they open. At 1235 the doors open but we wait for my eldest son and friend. He comes 1240 we all go in and we are seated on table 4 but eldest doesn't bring a friend. At 1310 we get Steak and Ale pie, new potatoes and garden peas plus Chocolate cake with fresh cream. Our host lie Rob Parker give the latest news and gives us team sheet. We are introduced to our guest Les Vinikolo a former player who talks about his life. After we had a quiz where our table came joint 1st. At 1445 we are invited to sit in our places to watch the match. As wheelchair user my place is in another bar where season ticket holders sit outside on their balcony. My place is outside and my guests sit below or stand next to me. At half time the score is 18 - 6 and the full time was 72 - 6. We went back into our room The Legends Suite and watched our host Rob Parker have an auction with hoodies that were from the players. Robbie Paul, the Chief Executive gave his overview of the match Later James Lowe gave his view of the match. Rob Parker thanked everyone and told us the Legends Suite is open next week when Bradford play Dewsbury. We got home at 1900. I put my feet up and watched TV.
08/06 In the morning I used my PC and upgraded my mobile to a Samsung Galaxy Young 2. I contact Yorkshire Police to asked if my report had been closed but instead I said it happens every day. After lunch I used my PC and emptied my top drawer to clean it out. I put my feet up and watched TV. I put my stuff back into my top drawer because my bowels had opened. I tolder carers who changed my pad. After tea I used my PC.
09/06 In morning I used my PC and my brother, Peter, gave me 3k towards my new Mobility WAV. After lunch I used my PC but at 1500 I went Seacroft Centre to shop at Poundland and Tesco's. When I got home I gave a carer my Vindiloo for my tea. After tea I used my PC.
10/06 I used my PC this morning. Brotherwood contacted me I told them I was waiting until somebody measured me from the floor to the top of my head = 140cms, I tried get a Samsung Galaxy Young 2 Pay and Go from O2 BUT my postcode doesn't work on their system. I got email from Leeds Citizens' Panel inviting me to a group discussion on 23/06 1330 in Leeds Civic Hall to discuss Statuary Notices 2015. I put a request to attend to my nurse. After lunch I used my PC. At 1500 the carers asked me if I would like to go to Roundhay Park. We got there at 1530 and went to the lake which we got about 100 yards but the path wasn't suitable for .wheelchairs. We fed the ducks and headed back and we got home at 1730. After I used my PC.
11/06 I used my PC and I ordered a Samsung Galaxy Young 2 from O2 Delivered tomorrow. I gave my list of my requests. After lunch I used my PC. At 14:30 I went outside to read my book. At 1530 I went to my room and I used my PC. After tea I tidy my basket and I used my PC.
12/06 I had my hair cut this morning before my shower. After breakfast I used my PC and got a text from Jacamo telling me my shorts will be delivered between 0830 - 0930. I started tidying my room. After lunch I used my PC, tidying my room and phoned O2 to asked them where is my Samsung Galaxy Young 2 is ? They told me the person delivered it delivered to no. 10 Naburn Walk ? They will arrange for it to be delivered to Ravensdale Nursing Home on Monday. My nurse asked me to give her a list of my appointments I gave her one for June - July. I was having difficulty with Blogger and I restarted my PC. After tea I cleaned my teeth and used my PC.
13/06 I had my weekly bladder wash and I used my PC. I ordered a grabber from Amazon. After lunch I used my PC. At 1430 I went to Tesco's in Seacroft centre I looked at Smart TVs and bought some shopping.
14/06 I started getting ready to go to Odsal to see Bradford v Dewsbury. I used my PC and at 1130 we, my carer, driver and me set off to Odsal. We arrived at Odsal at 1215, my driver went park my minibus and said outside The Conference and Banqueting Centre until he came back. We were seated on table no. 5 and the guests didn't arrive until 1330. We had home made Chicken Kiev, saute potatoes and green beans followed Chocolate Cheese Cake and after Rob Parker gave us the team news. We had quiz and at 1450 we were inviting to go to seats but it was raining and I decided to stay inside and watch the match in the viewing gallery. It was very close and at half time it was 0 - 6 and at the end it was 22 - 18. After the match we stayed while Jake Mullany got the man of the match. James Lowe describe his feelings and what he's going to do before next weeks match. After we went home at 1800 and got there at 1845. I ordered a 11 inch Chilli Pizza from Soprano's.
15/06 In the morning I got a texted from Tesco's Direct telling me my new smart TV would be here between 0700 - 1300. I booked a taxi/private hire car to take our OT, Emma, and me to Temple Newsham House at 1500 to the Leeds' Citizens Panel which starts at 1600 and I booked the same firm Blueline for the return at 1900. I used my PC before lunch. After I ordered 3 tickets for Bradford v Batley on the 28/06 at 1500 n the Conference and Banqueting Centre. I enquired if Hunslet have a similar centre at Parkside ? I used my PC to watched a movie before tea. After tea I used my PC.
16/06 I had problems with my Catherter this morning. I was by-passing but my nurse cleared it using a bladder wash. After breakfast I used my PC. After lunch I used my PC before going to Temple Newsham House at 1500. My taxi arrived at 1515 from City Cabs and we arrived at Temple Newsham House at 1450. At 1500 the councillors from East Leeds met for there meeting and this finished at 1600. We all went to another room to have sandwiched, flan, biscuits and tea/coffee. After we spilt in to our wards, my ward is Crossgates and Whinmoor. We debated litter and I raised Pavements but the chairman just took some notes. It finished at 1830, I emptied my bag and went outside to wait a City Cab. He came at 1900 and we got home at 1930. I ordered a Chicken Nawabi Khana with a Garlic Naan.
18/06 I used my PC and phone Tesco Direct to cancel my Smart TV. I arranged with o2 to return my new Samsung Galaxy Young 2 and the memory card because I can't work it. After lunch my new Smart TV arrived and I spoke with Tesco Direct to tell them. I put my feet up and watched TV. After tea I used my PC.
19/06 I ordered a new Volkswagen Caddy Life 1.6TD from Brotherwood in red this morning via Mobility. In the afternoon I asked a carer if she would file my fingernails. I sent a text to my brother and my 2 sons informing that I bought a new car. I used my PC and texted my eldest son to find out where we're standing on Sunday. After tea I used my PC.
20/06 I used my PC in the morning. After lunch I went to Tesco's to get some cash and I bought shower gel, Indian snacks and 6 bottles of water. After I got changed to go out to Seacroft Green Club. We set off because 1 of our Activities had organise an event for people who suffer from Hutchingson's Disease. We stayed there until 0030 and got home at 0100.
21/06 I used my PC in the morning until 1200 when my eldest son picked me up to take me to Pontefract races for my Father's Day present. There were 7 races and it finished 1715 but the traffic was very busy and got home at 1830. I used my PC to order a Pizza.
22/06 In the morning before I got up my Catheter became blocked my nurse tried a bladder wash it didn't work and she called ambulance 999 at 1000. We arrived St James at 1100 and into A and E to be examined. At 1600 I was admitted and my Catheter was changed. I went to Ward 27 firstly in a room for 30 minutes and to another room with 6 beds. I was in bed 25 where I asked where is my Air Filled Mattress ? They ordered 1 and it arrived later. I tried to get my TV working but Hospidia Helpline wouldn't answer they helpline. They answered it at 2010 I spent all the night watching TV. 23/06 In the morning the doctor discharged me but I had to wait for transport which arrive until 1630. I home at 1730, unpacked and asked a carer if I could have my Indian snacks. After I put my feet up and my TV.
24/06 In morning I bought 3 tickets to watch Hunslet v Bradford from Hunslet in the Parksiders Package. I emailed Hospidia to complain about their service. In the afternoon I used my PC and I had to restart my PC. After tea I used my PC to sell my Leedsware Bell on Ebay. I cleaned my teeth
25/06 I got a Jiffy Bag from O2 to return my Samsung Galaxy mobile and it's memory card. I used my PC but it was very slow and I restarted it. In the afternoon I went to Seacroft Village Hall, the Post Office and to Tesco's. After tea I cleaned my teeth and used my PC.
26/06 This morning I addressed the jiffy bag that O2 had sent me. I went on my PC before going to Coffee morning at 1130. After lunch I went to the Post Office to post the jiffy bag to O2. I put my feet up and watched my TV. Before tea the nurse told me I've got to book a taxi to take me to Odsal on Sunday.
27/06 I phoned Social Care Direct to complain about how Ravensdale has treated me over my agreed activity to go to Odsal tomorrow. I used my PC for the rest of the morning. In the afternoon I used my PC, put my feet up and watched TV.
28/06 The nurse told me I wasn't going Odsal because they short staffed. I sent an email to Social Care Direct telling that I want to Live Independently. After lunch I used my PC and my bowels opened. I told the carers who changed my pad. I put my feet up and watched TV. After tea I used my PC.
29/06 I used my PC this morning. After lunch I went to Tesco's for some shopping. The manager came to reimburse me for tickets to go see Bradford v Batley After tea I used my PC.

My Life being Disabled April 2015
01/04 This morning I asked the nurse what time is my appointment to St James. My appointment is at 0930 and it's something about my eyes that could last for 3 hours. I told her that I want to get up at 0630. For the rest of the morning I used my PC. My eldest son phoned to tell me that he's coming after lunch. He came at 1400 and left at 1545. After tea I used my PC.
02/04 I got up this morning because I had an appointment for 0930 at the eye clinic to get my eyes checked. My transport came at 0845 and we arrived at St James at 0915. I got called for a nurse to test eyes and to put drops in my eyes. We waited for a eye specialist. He tested my eyes and said they are OK. It was 1015 and went to the Patient Transport enquiry to let them know we had finished. Our transport arrived at 1100 and we got home at 1130. I used my PC to organise Asda to deliver my shopping tomorrow between 2100 - 2300. After lunch I asked the nurse to forward my requests to attend Wetherby races and to go to England v New Zealand on 01/11 at 1500 in Hull.
03/04 In morning I went to the coffee morning in the Activities room. After lunch my bowels opened my carers changed my pad. For the rest of the afternoon I put my feet up and watched TV. After tea I used my PC.
04/04 I used my PC in the morning. In the afternoon I went to the Activities room and failed trying to get their PC and Screen working both together. After I used my PC.
05/04 In the morning I didn't get up until 1200 because our hoist was broken. After lunch I used my PC. After tea I used my PC.
06/04 In morning I got ready to go to Tetley's Stadium to watch Dewsbury v Bradford at 1500. I used my PC before setting off to Tetley's stadium at 1245. We arrived 1330 and shown to the car park by a steward. I asked if this was the Disabled car park and he said it's full. We went to receipton and outside the car park was empty ! We went into the reception and walked through to the lift. This wasn't suitable for my electric wheelchair because my carer and driver had to walk up to floor 2. I had to travel by myself and couldn't reach the alarm also the lift doors closed when I was getting in. It was 1410 and the bar was on floor 1. My carer went to buy drinks and I noticed that the Disabled viewing platform only accommodated 4 wheelchairs with our carers sat behind. There was 2 disabled viewing platforms at the other side of pitch but I couldn't see any carers. A stand support stood right in front the Disabled viewing platform. Before the match started I went to the disabled toilet but this on the 1st floor. Which meant that I had go the lift again but when I got there it was locked and the key held the bar. I went to the bar but it was packed eventually I asked a person who was working if I could have the key. The toilet was O.K. but I had the problem again. I got back just for the kick-off. When play was round about the centre I couldn't see. It was really hot and with a crowd of over 4,000 it was full, mostly by Bradford fans. Dewsbury lost 16 v 30. It was chaos at the end. Trying to get out of the car park was very difficult. After we went to the Mermaid Fish restaurant. We got home at 2010.
07/04 I used my PC all morning and most of the afternoon.
08/04 I used my PC all morning apart when my GP came to look at my feet. In the afternoon I had feet put up and I watched TV. In the evening I used my PC.
09/04 In morning I went to Crossgates to get 2 photo for my passport. I went to Wilko's and bought 2 continental and went the Post Office to post it. After lunch I used my PC. After tea I used my PC.
10/04 In the morning I used my PC. In the afternoon I went Seacroft town centre and on the way back I reported cars parked on the pavement on Naburn Drive. After I used my PC.
11/04 I used my PC in the morning. After lunch I put my feet and watched TV but at 1600 my bowels opened and my carers changed my pad. After tea I used my PC.
12/04 I used my PC in the morning to investigate a holiday at the Confortel Fuengirola but the travel agent Hotels.com said there's nobody there. I will try tomorrow. In the afternoon I put my feet up and watched TV but at 1600 my bowels opened and my carers changed my pad. After tea I used my PC.
13/04 In the morning I used my PC and the unit manager informed me that nobody wanted to volunteer for a holiday in Fuengirola. In the afternoon I put my feet up and watched TV. I phoned Bradford and ordered tickets for the match Doncaster v Bradford on 26/04 at 1400. After tea I used my PC.
14/04 I asked my carers that I wanted to speak with Activities about my holiday in Fuengirola and getting Stevo to attend the Huntingdon's Disease Association Night on 20/06. After lunch I put my feet up and watched TV.
15/04 I phoned my Social Worker about me going abroad for a holiday - she said she's going to speak with the management of Ravensdale. In the afternoon I put my feet and watched TV. I used my PC after tea.
16/04 This morning I used my PC and replied to an email from Doncaster about hospitality packages. In the afternoon I attended the Carer's Forum from 1300 - 1500. After I went my room and used my PC.
17/04 This morning I found a restaurant in Doncaster that we can go to after match Halo Lounge. I emailed and find out it was suitable for wheelchairs. I booked a table for 3 on 26/04 1700. I received an email from Discovery Holidays giving me a quote of nearly 7k for me, 2 nurses and 4 carers to stay in Las Palmeras Hotel for bed and breakfast for 14 nights. I forwarded it to my brother. At 1200 I had my lunch because I had an appointment with Wheelchair Services to have a driving test. It lasted a hour but I passed. We came home and I used my PC. After I used my PC and I got a reply from my brother telling me to forward the email to Ravensdale manager.
18/04 In the morning I used my PC. After lunch I put my feet up and watched my TV. After tea I used my PC.
19/04 In the morning I tried to buy a new Bulls shirt but the phones were switched off. I used my PC before lunch. After I put my feet up and watched TV. After tea I used my PC and my wife phoned me.
20/04 I went to see the manager about my holiday Fuengirola and he's going to speak to my brother Peter. I'm using my PC. After lunch I put my feet up and watched TV but at 1500 Activities came to tell me there's a quiz in our lounge. It finished at 1630 and I went to my room. After tea I used my PC.
21/04 In the morning I used my PC and I bought a new green shirt from Bradford Bulls.
24/04 I got my new shirt from he Bulls and I went to the coffee morning. In the afternoon I phoned my Social Worker to countersign my application for a passport and left her a message. I used my PC and my bowels opened and my carers changed my pad. After tea my eldest son phoned he wanted to go to Doncaster on Sunday.
25/04 I used my PC in the morning. In the afternoon I put my feet up and watched TV.
26/04 I got ready to go to Doncaster, Keepmoat Stadium to watch Doncaster v Bradford at 1400. My eldest son borrowed a WAV for the day. We set off at 1215 and arrived at 1315. The Disabled car park was at the opposite end stadium and it took me 20 minutes to get there. We said we wanted to watch the match from the elevated viewing platform. The lift took us up level 2 and from the viewing platform could hold 5 wheelchairs with carers sat next to them. It was a good stadium, After the match my son took me home and I ordered nabew khana from Jinnahs.
27/04 In the morning I used my PC but when Activities asked me if I want to go to Tescos I said I wanted. We set off at 1130 and arrived 1230. We had lunch in the café in Tescos where I Lasagne with Garlic bread. We shopped in Tescos until 1430. We got home at 1530 and I used my PC.
28/04 In the morning I saw my GP about my leg and an itching head. He prescribed some tar shampoo. I used my PC for the rest of the morning. In the afternoon I put my feet up and watched TV. After tea I used my PC.
29/04 In morning we, myself and 2 residents with 3 carers went to Tesco's. We had lunch Tesco's café and went shopping. I bought 2 pairs of shorts, kitkats, a pizza cutter and some sugar free polos. After I put my feet up and watched TV. After tea I used my PC.
30/04 In the morning I had my monthly Care Plan review and I an email from the manager giving me costings for my holiday abroad. After I used my PC. After lunch I put my feet up and watched TV. After tea I used my PC.

The Disabled car park was at the opposite end stadium and it took me 20 minutes to get there. We said we wanted to watch the match from the elevated viewing platform. The lift took us up level 2 and from the viewing platform could hold 5 wheelchairs with carers sat next to them. It was a good stadium, After the match my son took me home and I ordered nabew khana from Jinnahs.
27/04 In the morning I used my PC but when Activities asked me if I want to go to Tescos I said I wanted. We set off at 1130 and arrived 1230. We had lunch in the café in Tescos where I Lasagne with Garlic bread. We shopped in Tescos until 1430. We got home at 1530 and I used my PC.
28/04 In the morning I saw my GP about my leg and an itching head. He prescribed some tar shampoo. I used my PC for the rest of the morning. In the afternoon I put my feet up and watched TV. After tea I used my PC.
29/04 In morning we, myself and 2 residents with 3 carers went to Tesco's. We had lunch Tesco's café and went shopping. I bought 2 pairs of shorts, kitkats, a pizza cutter and some sugar free polos. After I put my feet up and watched TV. After tea I used my PC.
30/04 In the morning I had my monthly Care Plan review and I an email from the manager giving me costings for my holiday abroad. After I used my PC. After lunch I put my feet up and watched TV. After tea I used my PC.

My Life being Disabled May 2015
01/05 I went to the coffee morning where I won a box of Milk Tray. I saw Emma the OT who's dealing with my holiday. In the afternoon I had my fingernails filed and I used my PC. After ea I cleaned my teeth and used my PC.
02/05 This morning I used my PC and I asked the nurse if I could go to Tesco's by myself this afternoon. After lunch I went to Tesco's by myself but while I was there the nurse phoned to tell me I didn't sign the form and she sent a carer to help me ! We came home, unpacked my shopping and the nurse explained the form and the consequences of me going out by myself.
03/05 In the morning I used my PC. After Sunday dinner I used my PC and at 1600 my bowels opened and my carers changed pad. After I used my PC.
04/05 The match Bradford v Halifax is postponed on 17/05 because Halifax are still in the Challenge Cup. I chosen Bradford v Doncaster on 07/06 instead. I asked my carers if I could go swimming but they told me to go see Activities.
05/05 There was a group of staff in the corridor this morning when I for breakfast. I thought I heard that a resident had died over night. I used my PC for the rest the morning. After lunch I saw my GP about my hernia in my stomach, my right leg and he said do my bowels work correctly and said no. He prescribed another drink to take daily. I got a quote from Saga for travel insurance of 30 pounds.
06/05 I emailed the Regional manager to find out what her Passport number is. For the rest of the morning I used my PC. In the afternoon watch 2 movies on my PC. The manager gave me the passport number for the Regional Manager. After I used my PC.
07/05 I went to vote in the school opposite but I couldn't in because the door into voting room was too small for my electric wheelchair. I complain Leeds City Council and copied it to Richard Burgon and Pauleen Grahame. After lunch my bowels opened and my carers changed my pad. I went shopping to Seacroft town centre. When I finished, I came home, went my room, unpacked my shopping and used my PC. After tea I used my PC.
08/05 In the morning I used my PC. I had lunch at 1200 because I had an appointment with the Podiarty at 1440. My transport came at 1300 but the driver looked at my wheelchair and decided it wouldn't in his vehicle. He contacted his control to get a larger vehicle. This came at 1400 but they decided their vehicle wasn't big enough. They didn't take me and I went inside to phone their Complaint Dept.. I tried from 1430 - 1530 but nobody answered and I sent them an email. After Disabledholidays.com emailed me to tell me no charter flights will allow me to take my wheelchair on their planes.
09/05 I used my PC this morning and I emailed my gp's secreraries about my medicines. After brunch I used my PC, put my feet up and watched a movie on my PC. After tea I used my PC.

19/05 I used my PC until Patient Transport came at 1030 to take to the dentist where I had an appointment at 1130. He tried my new teeth before he sent them away to be finished. He made another appointment on 02/06 at 1100. While waiting for our transport I made an order for Asda home delivery for 20/05 at 1500. We got home at 1330 and I had lunch. After lunch I put my feet up and watched TV. After tea I cleaned my teeth and put together my mini TV.
20/05 This morning the nurse reported my electric wheelchair to Wheelchair Services because my right footrest wouldn't go down and my seat was cracked. I went on my PC. After l put my legs up and watched TV. After tea I went on my PC.
21/05 I used my PC in the morning. In the afternoon I had my hair cut. I put my feet up and watched TV. After tea I used my PC to order a pair of Chinos from Jacamos.
22/05 In the morning I went to the Services Users meeting in the Activities room. It was chaired by Emma Simpson the OT and we discussed the chair for the meeting which I volunteered. Terri on Activities is leaving at the end of May. I suggested her replacement should be able to drive. Activities ad already the new handiman if he would drive. Why do I have to wait for my prescriptions ?? I'm waiting for my Zinc and Castor Oil and my Sycno bags fore more than weeks. I told the meeting I want to go to Franco's for meal and Hiedi said to see her later. After lunch I emailed Franco's to ask if there any hazards for wheelchair users ? They replied saying wheelchairs can use Franco's.
23/05 During my bladder wash my catheter became blocked and 3 nurses changed it with difficulty. After breakfast I used my PC. After lunch I put my feet and watched rugby league on my TV. After tea I cleaned my teeth, put my feet and watched my TV. I watched Bradford beat Halifax 18 - 4.
24/05 I went on my PC in the morning. After Sunday lunch I put my feet up and watched Rugby League on TV. After tea I used my PC.
25/05 The nurse remaindered me that I have my Care Plan review on Wednesday. She asked a carer to help me with my Property list. I used my PC and helped my carers with my property list. After lunch I used my PC. Later I put my feet up and watched TV. After tea I used my PC.
26/05 My nurse told me that my bladder washes were being reduced to 1 per week. After breakfast I used my PC. My GP came at 1200 and I told him about my red leg he prescribed antibiotics. I told him I new stockings, Zinc and Castor Oil, a new splint for my right arm and need more Paracetamols and he said he'll speak to our nurse. After lunch I used my PC.
27/05 This morning I used my PC before attending my monthly Care Plan review. At 1130 I met with my Advocat and my named nurse After lunch I put my feet up and watched TV. After tea I used my PC.
28/05 This morning I used my PC. I organised the driver, my carer, my eldest son and his wife and me to go to watch Bradford next Sunday 07/06 with a meal. In the afternoon I went to Tesco's and I back at 1530. I unpacked my shopping, used my PC and sent an email to Richard Burgon MP copied to Pauleen Grahame councillor about the footpaths. After tea I used my PC.

My Life being Disabled March 2015
01/03 I got ready to go to Featherstone because Bradford are playing there this afternoon. We set off at 1300 and there at 1400. The traffic was very busy and my driver asked steward where's the disabled parking. He said it's full and indicated to park in the normal spaces. We parked our minibus and asked a steward where s the wheelchair entrance for away supporters. He pointed to more Bradford supporters in wheelchairs and told us to wait with them. Another steward took us at the side of the stand where there was an old elevated viewing platform. It was not under cover, full of tables and plastic chairs with no seats for our carers to sit on. We found out that away wheelchair supporters should go to a new stand with a covered elevated viewing platform which was empty. Another 2 Bradford wheelchair supporters joined and weather got worse. In the end I told my carer I want to go home at 1445. We went back to our minibus and got home at 1545. I put my feet up and watched TV.
02/03 In the morning I used my PC to change my password plus I emailed Sarah Williams at the and in the afternoon I put my feet up and watched TV.
03/03 My nurse changed Catheter this morning. After I used my PC and replied to Sarah Williams to send my email to Featherstone. After lunch I put my feet up and watched TV. At 1600 my bowels opened and my carers changed my pad plus my shirt. After tea I went on my PC.
04/03 I got dressed up because I'm going to Maxis Chinese Restaurant n Leeds and the table is booked for 1900. Unfortunately at 1600 I had a terrible stomach ache and it lasted all evening. I told the nurse and 1730 I texted my sons to tell them I won't be going. I put my feet up and watched TV. I realised that I had Constipation because my bowels hadn't been open for at least 2 days.
05/03 In morning I used my PC. At 1130 I attended my monthly Care Plan review which lasted until 1230. In the afternoon I put my feet up and watched TV.
06/03 I was suppose to go to Crossgates in the Access bus but I was constipated and felt ill. I stayed in bed all morning. I got up in the afternoon, put my feet up and watched TV.
07/03 I worked on my PC in the morning. After brunch I put my feet up and watched TV.
08/03 Got ready to go to Odsal to see Bradford v Hunslet. We are setting off at 1130. We arrived at 1315 due to Satnav. We went into the Sponsors stand to have our 2 course meal which was Chicken stew, dumpling and chocolate cake. After we went into the viewing gallery to watch the match. At half time coffee and biscuits were served At full time we left missing man of the match. We got at 1800, I put my feet up and watched TV.
09/03 I had to get up at 0700 but the night forgot! I woke up at 0815 and told the carers I had an appointment at the dentist at 1125. I got a bed bath, cleaned my teeth and dressed. After breakfast my transport came and we got to the dentist at 1030. At 1120 I went in his surgery where he trimmed the mould for my new top set of false teeth. This finished at 1200 and we went downstairs to wait for our transport. It arrived at 1345 and we got home via LGI at 1430. I had my lunch, went to my room and used my PC.
10/03 In the morning I used my PC. I had lunch at 1200 because I had an appointment at 1430 to get my Baclofin pump filled up. After lunch my bowels opened and my carers changed my pad. My transport came at 1330 and we arrived at Pinderfields at 1410. At 1420 I saw the consultant who filled my Baclofin pump up. The nurse told me the of my next appointment 14/08. We went to the waiting room but we had to wait until 1645 for transport. When got home I had tea and after I told the nurse the date of my next appointment, I have cancelled Access bus to Crossgates, asked if she's going to change my Care Plan about changing my Catheter bags and would she tell Activities that I want to go to Leeds/Bradford Airport followed by Fish and Chips at Murgatroyds in Yeadon. I went to my room and logged on to my PC.
11/03 I didn't get up until 1030 because nobody put my shower sling in the wash. After breakfast I used my PC. After lunch the nurse filed my finger nail. I went on my PC.
12/03 In the morning I used my PC. In the afternoon I went with Activities to my GP to pick up some prescriptions, to Tesco's shopping where I bought a mains extension and some Kit Kats. After tea I used my PC.
13/03 I went to the Coffee morning where we played Bingo and 2 Quizzes. After lunch I used my PC.
14/03 I got an email from First Direct Arena asking my requirements are when I go to see Paloma Faith next Saturday.21/03 at 1930. I told them that a wheelchair space and a seat next to me for my carer. I asked them what time does it finish ? Emailed Dewsbury Rams to ask if they have a package that includes a meal ? In the afternoon I put my feet up and watched TV.
15/03 My Air Condition Unit that I ordered on Weds 11/03 arrived on Thurs is waiting to be PAT tested with the handyman ! After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.
16/03 In the morning I used my PC. In the afternoon I used my PC, put my legs up and watched TV. After tea I used my PC.
17/03 In the morning my son, Andrew, came at 1130 but he had to leave because lunch was at 1230 because we're having afternoon tea at 1500. After lunch I used my PC. At 1500 I went to the Activity room for Coffee and Cakes where we had 2 quizzes.
19/03 I phoned AB Cars to confirm that they had a booking on Saturday at 1800 to me and carer to First Direct Arena and to return at 2300. I asked the carers if I could go to Tesco's and they said this afternoon. I used my PC. In the afternoon we went to Tesco at 1430, We got home at 1600, unpaked my shopping and I went on my PC.
20/03 This morning our Clerical asked me to transfer #70 and she will give #70 in cash. The handyman started to set my ACU up. I used my PC. In the afternoon I put my feet up ad watched TV. After tea I went on my PC.
21/03 This morning I got ready to see Paloma Faith at the First Direct Arena. I went on my PC. In the afternoon I put my feet up and watched TV. After tea I get ready because our taxi from AB Cars was coming at 1800. He arrived at 1755 and we got there at 1830. We took the lift to floor(2) where the wheelchair users sat at front. There was about 50 spaces with chairs next to spaces for carers. It was a good view. The first act came on at 2000 Vintage Trouble and played until 2030. Paloma Faith came on at 2100 and she finished at 2300. Our taxi was outside and got at 2330.
22/03 This morning I used my PC. At 1100 I felt sick and I put my feet up and watched TV. I didn't any lunch. I had tea in my room.
23/03 I bought a CD/Radio/Alarm Clock with a remote control. I phoned Bradford Bulls and ordered 1 wheelchair space, 1 carer, 1 adult and disabled parking space for the match at Dewsbury on Easter Monday.. I'm trying to do my Family Tree but I'm finding it difficult. After lunch my bowels opened and my carers changed my pad. I put my feet up and watched TV.
24/03 This morning I used my PC and I got my CD/Radio/Alarm Clock. In the afternoon I sent it to be PAT tested. I put my feet up and watched TV. After tea I used my PC.

27/03 Yesterday I stayed in bed until after lunch. I got up, put my feet up and watched TV. Today in the morning I used my PC. After lunch my GP to look at my legs that were red, scabby and swollen. She said it was Cellulitus and prescribe some anti-botics. She said I must keep my feet up.
28/03 In the morning I used my PC to order a Kidney bowl from Amazon. On Ebay I put 2 bids on for Proof Gold Sovereigns.
29/03 My bids on Ebay were out bid and in the afternoon I put my feet up plus I watched TV. At 1600 my bowels opened and my carers changed my pad. After I went on my PC working on my Family Tree.
30/03 I asked the carers if they would get some batteries for my remote controls. I find out that Activities were taking me to Dewsbury on Easter Monday. During the morning I put my feet up and used my PC. After lunch I put my feet up and watched TV. At 1600 my bowels opened and my carers changed my and polo shirt. They found another sling because mine was soiled. I told my nurse that since I on antibiotics my sling was soiled twice. She agreed and I'm going to see the physoxxxx when she returns. Also I'm going to see Activities to see if we can go to The Mermaid Fish restaurant after the match on Easter Monday. After tea I used my PC.
31/03 My GP came this morning to look at my Cellulites and he prescribed something to clear the wax in my ears. In the afternoon I phoned my wife because she wrote a cheque from my pension. For the rest of the afternoon I used my PC. After tea I used my PC.

After breakfast my transport came and we got to the dentist at 1030. At 1120 I went in his surgery where he trimmed the mould for my new top set of false teeth. This finished at 1200 and we went downstairs to wait for our transport. It arrived at 1345 and we got home via LGI at 1430. I had my lunch, went to my room and used my PC.
10/03 In the morning I used my PC. I had lunch at 1200 because I had an appointment at 1430 to get my Baclofin pump filled up. After lunch my bowels opened and my carers changed my pad. My transport came at 1330 and we arrived at Pinderfields at 1410. At 1420 I saw the consultant who filled my Baclofin pump up. The nurse told me the of my next appointment 14/08. We went to the waiting room but we had to wait until 1645 for transport. When got home I had tea and after I told the nurse the date of my next appointment, I have cancelled Access bus to Crossgates, asked if she's going to change my Care Plan about changing my Catheter bags and would she tell Activities that I want to go to Leeds/Bradford Airport followed by Fish and Chips at Murgatroyds in Yeadon. I went to my room and logged on to my PC.
11/03 I didn't get up until 1030 because nobody put my shower sling in the wash. After breakfast I used my PC. After lunch the nurse filed my finger nail. I went on my PC.
12/03 In the morning I used my PC. In the afternoon I went with Activities to my GP to pick up some prescriptions, to Tesco's shopping where I bought a mains extension and some Kit Kats. After tea I used my PC.
13/03 I went to the Coffee morning where we played Bingo and 2 Quizzes. After lunch I used my PC.
14/03 I got an email from First Direct Arena asking my requirements are when I go to see Paloma Faith next Saturday.21/03 at 1930. I told them that a wheelchair space and a seat next to me for my carer. I asked them what time does it finish ? Emailed Dewsbury Rams to ask if they have a package that includes a meal ? In the afternoon I put my feet up and watched TV.
15/03 My Air Condition Unit that I ordered on Weds 11/03 arrived on Thurs is waiting to be PAT tested with the handyman ! After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.
16/03 In the morning I used my PC. In the afternoon I used my PC, put my legs up and watched TV. After tea I used my PC.
17/03 In the morning my son, Andrew, came at 1130 but he had to leave because lunch was at 1230 because we're having afternoon tea at 1500. After lunch I used my PC. At 1500 I went to the Activity room for Coffee and Cakes where we had 2 quizzes.
19/03 I phoned AB Cars to confirm that they had a booking on Saturday at 1800 to me and carer to First Direct Arena and to return at 2300. I asked the carers if I could go to Tesco's and they said this afternoon. I used my PC. In the afternoon we went to Tesco at 1430, We got home at 1600, unpaked my shopping and I went on my PC.
20/03 This morning our Clerical asked me to transfer #70 and she will give #70 in cash. The handyman started to set my ACU up. I used my PC. In the afternoon I put my feet up ad watched TV. After tea I went on my PC.
21/03 This morning I got ready to see Paloma Faith at the First Direct Arena. I went on my PC. In the afternoon I put my feet up and watched TV. After tea I get ready because our taxi from AB Cars was coming at 1800. He arrived at 1755 and we got there at 1830. We took the lift to floor(2) where the wheelchair users sat at front. There was about 50 spaces with chairs next to spaces for carers. It was a good view. The first act came on at 2000 Vintage Trouble and played until 2030. Paloma Faith came on at 2100 and she finished at 2300. Our taxi was outside and got at 2330.
22/03 This morning I used my PC. At 1100 I felt sick and I put my feet up and watched TV. I didn't any lunch. I had tea in my room.
23/03 I bought a CD/Radio/Alarm Clock with a remote control. I phoned Bradford Bulls and ordered 1 wheelchair space, 1 carer, 1 adult and disabled parking space for the match at Dewsbury on Easter Monday.. I'm trying to do my Family Tree but I'm finding it difficult. After lunch my bowels opened and my carers changed my pad. I put my feet up and watched TV.
24/03 This morning I used my PC and I got my CD/Radio/Alarm Clock. In the afternoon I sent it to be PAT tested. I put my feet up and watched TV. After tea I used my PC.
25/03 This morning my eldest son came at 1130 and stayed until 1230 and after I used my PC. In the afternoon I put my feet up and watched TV.
27/03 Yesterday I stayed in bed until after lunch. I got up, put my feet up and watched TV. Today in the morning I used my PC. After lunch my GP to look at my legs that were red, scabby and swollen. She said it was Cellulitus and prescribe some anti-botics. She said I must keep my feet up.
28/03 In the morning I used my PC to order a Kidney bowl from Amazon. On Ebay I put 2 bids on for Proof Gold Sovereigns.
29/03 My bids on Ebay were out bid and in the afternoon I put my feet up plus I watched TV. At 1600 my bowels opened and my carers changed my pad. After I went on my PC working on my Family Tree.
30/03 I asked the carers if they would get some batteries for my remote controls. I find out that Activities were taking me to Dewsbury on Easter Monday. During the morning I put my feet up and used my PC. After lunch I put my feet up and watched TV. At 1600 my bowels opened and my carers changed my and polo shirt. They found another sling because mine was soiled. I told my nurse that since I on antibiotics my sling was soiled twice. She agreed and I'm going to see the physoxxxx when she returns. Also I'm going to see Activities to see if we can go to The Mermaid Fish restaurant after the match on Easter Monday. After tea I used my PC.
31/03 My GP came this morning to look at my Cellulites and he prescribed something to clear the wax in my ears. In the afternoon I phoned my wife because she wrote a cheque from my pension. For the rest of the afternoon I used my PC. After tea I used my PC.

01/12 I phoned Leeds RL to buy a wheelchair space, a carer and disabled parking space for the match against Wakefield on Boxing Day which starts at 11:30. In the afternoon I, my driver and my OT went to Wheelchairs Services to pick up my new pink electric wheelchair. After tea I started changing my passwords.
02/12 I went to my Advocacy headquarters A4MHD in Cross Green because it was their Open Day. It started at 11:30 and we left at 13:30. On the way back we went B & M and Asda. We got home at 15:30.
03/12 I asked the nurse if I could go to The Championship Summer Bash held on 23/24 May in Blackpool. I finished changing my passwords. I got my tickets for The Boxing Day match between Leeds v Wakefield which starts at 11:30. In the afternoon I put my feet up and watched TV. At 15:30 my eldest came and me if I wanted to go to the German market in The Millennium Square. He said he'll ask the nurse.
04/12 I asked the nurse because she wasn't there when I went out. I used my PC before lunch. After lunch I put my feet up and watched TV. After tea I used my PC.
05/12 In the morning I went to the quarterly Care Plan review meeting where Ravensdale Nursing Home, my Advocate and myself discussed my Care Plan and it lasted 30 minutes.. In the afternoon I put my feet up and watched TV.
06/12 This morning I went to Tesco's for some shopping. When we came back I had Mulligatawny soup for lunch. In the afternoon I used my PC and watched TV.

08/12 Overnight I restarted my PC. This morning I used my PC, activities came to tell we're booked After in at The Beehive tomorrow at 12:30 and confirmed they are taking me to Leeds city centre at 11:00. I txtd Andrew to let him know. After lunch I used my PC, put my legs up and watched TV.
09/12 I went to the Beehive in Thorner for Xmas lunch. I came home and used my PC.

13/12 I filled a questionnaire in for Sky. In the afternoon I went to Tesco's for some shopping. We went in our minibus.
14/12 Last evening I went to bed early because I said I felt sick and this morning I missed church because the carers left me in bed until 11:00. After Sunday lunch I used my PC and I put my feet up and watched TV.
15/12 I got up at 07:00 because I had an appointment at 10:40 with the Orthotics at Pinderfields to try my new shoes on. My transport arrived at 09:00 and arrived at Pinderfields at 09:40. We called in at 10:30 and were finished at 10:40. We told the receptionist we were ready for transport but it arrived 12:30. We boarded but they didn't have extension leads for my safety belt but the driver moved me to accommodate this. We got home at 13:15. After lunch I put my feet up and watched TV.
16/12 I used my PC and started to tidy my room. I phoned Leeds RL to ask for a Disabled Parking space but they told me to email stephiewright@leedsrugby.com who deals with the requests. I tried but I didn't get through. After lunch I put my feet up and watched TV.
18/12 This morning I got up at 07:00 because I had an appointment at St James about my eyes at 11:00. My transport arrived at 09:00 and we arrived at St James at 09:40. We were called in at 10:50 and my were tested. I was informed that my eyesight was good enough to drive my electric wheelchair as Wheelchair Services had asked. We told the receptionist to book our transport home at 11:00. The transport came at 13:00 and we arrived home at 13:30. After lunch I put my feet up and watched TV.
19/12 In the morning I went to a Coffee morning where I won a mug playing Bingo. In the afternoon I put my feet up and watched TV. I set my PC to reset because it running slow.
20/12 In the morning I used my PC. In the afternoon I put my feet up and watch TV.
21/12 I had asked to get up 06:30 but my sling was wet so I got up at 07:30 to go to church. The access bus came at 09:20 and we arrived at 09:40. The service lasted until 11:30 and we got home at 11:50. In the afternoon my sister an her husband came. They brought me a Hyacinth. After I used my PC.
22/12 In the morning I used my PC and got a Xmas card from my MP. I also got a letter from Leeds MDC about my name and my address to which I replied. This afternoon I went to Tesco's with 2 other residents. We set off at 14:00 and returned at 16:30.
24/12 I got a letter this morning about Bowel cancer, asking me had I done the test. I had pass it on to the nurses and the centre hadn't received it. I gave it to the manager to deal with. Later the nurse came to tell me if my bowels open I should tell my carers the nurse has to examine it. After lunch I phoned the HC1/2 helpline to order a new HC1/2 form. I put my feet and watched TV.
25/12 My sons came with their girlfriends n Xmas day morning. We had Tomato soup, Turkey etc. and Xmas pudding for lunch. After lunch I put my feet up and watched TV.
26/12 I went to watch Leeds v Wakefield which started at 11:30. We got home at 14:30 and I put my feet up and watched TV.
27/12 In the morning I used my PC. After lunch I put my feet up and watched TV.

We got home at 1100. My eldest son came at 1200 and left at 1300. After lunch I used my PC, put my feet and watch TV. After tea I used my PC.
12/02 I sent email to the Practice Manager for my GP complaining about yesterday. I emailed Bradford Bulls to find out how much is cost to get in the Coral stand. In the afternoon I put my feet up and watched TV.
13/03 I went to see my Neurologist to have my 6 monthly check. After she said we'll change it to yearly. After lunch I used my PC. Activities held a quiz for the Service User.
14/02 This morning I used my PC and I bought a Sovereign from The Gold Bullion Co. I sent email to PALS asking to get a copy of all health records. In the afternoon I watched Catalans beat Castleford 13 - 12.
15/02 I went to Leigh to watch Bradford. We set off at 1215 and arrived 1315. We went into Morrisons and had a Big Breakfast. After we went in the ground but should gone upstairs viewing platform but the stewards to us to pitch side wheelchair users. Leigh won 36 - 24. When we got home I ordered a Nawebi Khana with Garlic Naan from Jinnah.
16/02 I went to my dentist in Chapellerton this morning. After lunch I watched a movie and tea used my PC.
17/02 I sent Sarah Williams my report from Leigh. I tried to get tickets for the Featherstone away but Bradford didn't have any and Featherstone said they have sent Bradford their allocation. I asked the nurse if I could go Maxi's at 1900 on 04/15. At 1330 the Wheelchair repair came and temporarily fixed my wheelchair. I used my PC for the rest of the afternoon.
18/02 I got my 2015 Gold Sovereign this morning. Later I got my flat cap from Amazon. I asked the carers if I could go to Tesco's.
19/02 We had a quiz in the morning and I won. In the afternoon I phoned Bradford Bulls and asked them if they were selling tickets for the Featherstone match - No. In the afternoon I used my PC.
20/02 In the morning I went to Crossgates shopping. After lunch I used my PC but I couldn't access Santander. I phoned them to find out I never changed my address. I watched a movie for the rest of the afternoon.

24/02 The Wheelchair repairs are coming this afternoon. I was suppose to have a bladder wash but the nurse told me she didn't think it was necessary. She's going to speak with my GP. The Wheelchair repair man came and fix my wheelchair. I had a bladder wash at 23:00 when I went to bed.
25/02 I got a letter from my GP's Practice manager about a mix up when I went to the Physio but I should got a home visit. In the afternoon I put my feet and watched TV.

17/02 I sent Sarah Williams my report from Leigh. I tried to get tickets for the Featherstone away but Bradford didn't have any and Featherstone said they have sent Bradford their allocation. I asked the nurse if I could go Maxi's at 1900 on 04/15. At 1330 the Wheelchair repair came and temporarily fixed my wheelchair. I used my PC for the rest of the afternoon.
18/02 I got my 2015 Gold Sovereign this morning. Later I got my flat cap from Amazon. I asked the carers if I could go to Tesco's.
19/02 We had a quiz in the morning and I won. In the afternoon I phoned Bradford Bulls and asked them if they were selling tickets for the Featherstone match - No. In the afternoon I used my PC.
20/02 In the morning I went to Crossgates shopping. After lunch I used my PC but I couldn't access Santander. I phoned them to find out I never changed my address. I watched a movie for the rest of the afternoon.
21/02 In the morning I used my PC. In the afternoon I put my feet and watched TV.
22/02 I used my PC in the morning. I put my feet up and watched TV in the afternoon.
23/02 I went to the dentist this morning. He filled a tooth and took an impression of my uppers. He reminded me that my next appointment was 09/03 at 1100. After we waited 3 hours for our transport. We went home and got home at 1545.
24/02 The Wheelchair repairs are coming this afternoon. I was suppose to have a bladder wash but the nurse told me she didn't think it was necessary. She's going to speak with my GP. The Wheelchair repair man came and fix my wheelchair. I had a bladder wash at 23:00 when I went to bed.
25/02 I got a letter from my GP's Practice manager about a mix up when I went to the Physio but I should got a home visit. In the afternoon I put my feet and watched TV.
26/02 I used my PC all morning. In the afternoon I put my feet u and watched TV.
27/02 I went to the Coffee morning in the Activities room where the cook talked to the residents about the new menu. After we listened to music. After lunch I went on my PC and I put my feet up while watching TV.
28/02 I used my PC to change my passwords in the morning. In the afternoon I watched Catalan beat Warrington 38 - 18.

My Life being Disabled - Jan 15
01/01 Changing my passwords today but I'm having difficulty.
02/01 Went to Crossgates shopping centre via the Access bus in the morning. I:00n the afternoon I put my feet up and watched TV. In the evening about 18:00 I felt funny, I asked the carers could go to bed at 21:00. At 21:30 I told the nurse that I felt better and she told my GP. I feel asleep and later.
03/01 Tried to connected my PC to my TV but failed. I used my PC all morning. In the afternoon I connected my PC with my TV. I put my feet up and watched TV.
04/01 I went to St. James church in the morning using the Access bus. After lunch the nurse tidied my room. After I put my feet up and watched TV.
05/01 A carer cut my hair and I asked another to book an appointment for the podiatry ASAP.
06/01 In the morning I used my PC and in the afternoon I put my feet up and watched TV.
07/01 After breakfast I was taken to St James A and E because my right hand was severely bruised. They took several X-rays and the doctor said it wasn't broken, They bandaged my right elbow and put my right in a sling. We came at 14:30, I had a Tuna sandwich and went to my room. I tidied it and logged on to my PC.
08/01 I emailed the manager of Ravensdale to complain the my washing is getting shrunk and 6 my hankies are pink. I organised a demo on a Citroen Berlingo Multispace with Automotive Group National for 11:00 on 26/01/15.
09/01 I got up at 07:00 but when I was being hoisted my bruised right started hurting. The carers put me back into bed and I asked the carers to phone Access bus to tell them I wouldn't be going to Crossgates. I got up at 10:00 and I had breakfast. I tidied my room and used my PC. I got the footstool from Amazon. In the afternoon I put my feet up and watched TV.
10/01 I used my PC in the morning. I ordered a Duronic HDM1 splitter, an aerial lead and 4 HDM1 leads from Amazon. I put my feet up and watched TV in the afternoon. After tea I went on my PC.
11/01 I got up at 07:20 because the carers were busy. I didn't have any breakfast and only half a cup of coffee. We to St James church for Holy Communion and we got home I put my sling on. I went on my PC and I asked if I could to watch Shaw Cross v York Acorn on 31/01/15 at 14:00.
12/01 In the morning I used my PC. In the afternoon I used my PC until 16:00 when I went down to the other unit where we watched a woman sing songs that were popular during World War 2. It lasted about 1 hour. I had Salmon fillet for tea.
13/01 In the post I got my disabled parking pass for the match Leeds v Bradford 25/01 at 15:00 which is Jamie Peacocks Testimonial. I used my PC in the morning. In the afternoon I used my PC, put my feet up and watched TV.
14/01 I got a HDM1 switcher and a TV Aerial lead from Amazon this morning. I emailed the manager of Ravensdale about a new service user who keeps coming into my room, I asked him when somebody is going to check my possession, can I pay Ravensdale monthly instead 4 weekly and do you have any procedures that cover a service user getting a Mobility WAV. In the afternoon I used my PC and I went to Thorpe Park Clinic near Colton to get my Hearing Aids repaired. From 18:15 to 23:00 the new service user kept trying to get in my room.
15/01 I was woken by him at 06:00 trying my door again. I used my PC and the new service user kept coming in my room. After lunch my wife phoned. I used my PC all afternoon. My prescription for my dressing arrived for my arm and the nurse fax it to Boots.
16/01 I went to Crossgates shopping in the morning. After lunch I used my PC.
17/01 Last night the Service User kept banging on my door until 22;00. This morning he keeps coming into my room. I reported this to Social Services Direct. After lunch I used my PC and the cook asked if I wanted my Pizza with Bolognese sauce. I used my PC still being disturbed by Service User. After tea I used my PC.
18/01 I went to church this morning and after lunch I used my PC.
19/01 I emailed the secretary of the Leeds East Killingbeck and Seacroft branch of thr Labour Party to find when have meeting, where they are and is wheelchair accessible. He emailed me back and the next meeting is tomorrow starting at 1900. In the afternoon I used my PC.
20/01 I used my PC and order a home shirt plus an away shirt from Bradford Bull shop. In the afternoon I spoke to Sky to get my Sky TV, phone and broadband moved from room 19 to room 10 on 23/01.
21/01 My phone was disconnected this morning. After tea I used Activities connection.
22/01 After tea I used Activities connexion. I booked a table for 6 at Maxis Chinese restaurant for next Weds at 1830.
23/01 Sky came to move Sky TV to room 10. I went to Crossgates shopping and when I came home all my possessions had been moved into room 10.
24/01 I got a text from the car dealer to ask me that I will give him prove of DLA Benefit and driving licences of 2 drivers. I went to the Activities room to use their Broadband. In the afternoon I put my feet and watched TV. After tea I used my PC until 1800.
25/01 This morning I went to St. James church in Seacroft and after lunch I went to watch Leeds v Bradford 34 - 14. When I got home I ordered a Chicken Nabawi Khana from Jinnahs York Road.
26/01 I met the rep from mobility with a Citreon Berlingo but it too small he told me they have bigger one which costs 500 pounds more and we agreed to meet on 04/03 at 11:00. After lunch I put my feet and watched TV. After tea I used my PC.
27/01 I used my PC in the dining room.
28/01 I used my PC in the dining room and after lunch I put my feet up and watched TV.
29/01 I used my PC in the quiet lounge.
30/01 I didn't go to Crossgates with the Access bus because I couldn't get up the ramp. The nurse called my GP because my Cellulitus was flaring up. He came in the afternoon and presciped some anti-bioptics.
31/01 I went to Activities room to use my PC in the morning. I put my legs up and watched TV in afternoon and the evening.
