04/06 I had a wash in bed this morning because my Cellulitiis was getting worse again and I noticed I had no Shower Gel. After breakfast I went to Tesco's and bought 3 bottles. 05/06 I used my PC before getting ready to go Odsal where Bradford are playing Dewsbury k.o. 1500. Tony my driver/carer came at 1130 and we got to Odsal at 1230. We went into CTF Legends Suite where I bought drinks. At 1300 Robbie Hunter-Paul went on the stage, as compere and told ever body the itinerary for this fternoon. At 1320 our meals were served, lemon Chicken, roast potatoes and pickles. After the 1st course Robbie introduced a old player Wayne Goodman who played for several teams in Super League. After the 2nd course, Strawberry Cheesecake, he held a quiz. For the match we went outside because it was sunny. At half time it was 6 - 14 and at full time it was 12 - 18. We came home because too warm to go inside. When I got home I order a 11in Chilli Pizza with Pepperoni which I had in the dining room.
06/06 I phoned Social Care Direct this morning but they didn't answer about my Independent Living. I used my PC and email our OT to ask her if I can go to Oldham on 15/06, Leigh on 03/07 and Featherstone on 24/07. In the afternoon my bowels opened and my carers changed pad. I went in the gardens where I put my feet up to read a book. At 1700 I had my tea. I went to my room, cleaned my teeth and used my PC.
07/06 This morning I tried to use my PC but my Windows upgraded and it 2 hours. I tidied my room and went out the garden. At 1130 I went back to room and my landline started to ring. It was Social Care Direct who told me their delaying with my request and I should have a Social Worker in a few weeks. My PC had finished upgrading and I use my PC. In the afternoon I went in the garden sunbathing until 1500 and I went to my room. I used my PC. After tea I used my PC.

09/06 During my bladder wash, which didn't work I felt my catheter was blocked. I told the nurse and she told another nurse who said she'll change my catheter. 10/06 I had a meeting about my Activity time and Scott the acting manager told I only 4 hours per week. We sorted my visits to Leigh and to Featherstone but we didn't sort anything else. 14/06 The nurse told me my Quarterly Care Review is being held at 1100 today.I used my PC, I asked if the office would buy some flowers for the Lady Mayoress and for Councillor Pauline Grahame for the Open Day on Friday. I decided to load Windows 10. I had my Care Plan Quarterly Review. After Brunch I put my feet up and used my PC. I ordered some shopping from Asda. I watched my TV but at 1630 I got an email from the councillor to say she can't come on Friday. I put my feet down and I told the office. I used my used my PC. After tea I used my PC.

16/06 I phoned the Co-op bank to find out why somebody has set up a direct debit to Camelot Group plc A00151/2991. The Co-op bank deleted and I sent email to them. After lunch I put my feet up, watched England beat Wales 2 - 1.I phoned Bradford Bulls for tickets Leigh away. I used my PC. After tea I cleaned my teeth and used my PC.
17/06 I went to the Coffee Morning this morning after I used my PC. After lunch I used my PC and at 1430 I went to watch Elvis in our TV lounge. He played until 1520 when the Lord Mayor, Lady Mayoress and local councillor Pauline Grahame with drivers. They stayed in the garden until 1600 and they went upstairs into the Activity room for sandwiches etc. After this they went to the Civic Hall. After tea I used my PC.
18/06 I used my PC and I found out the freezer in the kitchen had defrosted. I phoned my youngest son via Facebook in Tenerife. After Brunch I used my PC and put my feet up. After tea I cleaned my teeth and used my PC.
19/06 I got ready to watch Bradford v Batley from the Conference and Banqueting Centre in Odsal We set off at 1130, my carer/driver and I, arrived at Odsal at 1215. We were allocated on table 2 but the room was filled with 19 tables. The time there were 9 tables but today is Father's Day. On the menu the main course was Pork Snhnitzel with new potatoes and Mushroom sauce. After the speeches from Old Players we had Trio Chocolate Delice. After quiz we went outside to watch the match. Although it was raining I keep dry on the back step. At half time it was 10 - 6 and in the 2nd half in a very close match Bradford got a drop goal 3 minutes from the final whistle winning 17 - 16. We got home at 1815 and I ordered a Chilla Pizza.
20/06 I went to Seacroft hospital because I had an appointment at 0915 to get my new splint. When I got at 1030 I used my PC. Before lunch my eldest son with his Fiancee after going to Saltmarshe Hal where he's planning to hold his Wedding reception after their Wedding in Howden Church. After lunch I put my feet up and used my PC. A carer came to tell me Activities a holding a movie show at 1500. I went and the movie was Daddy's Home. After tea I used my PC.

At 1515 my bowels opened and my changed me because I was going to the Bullbuilders Race Night at Odsal after tea. After tea we went for some diesel before going to Odsal although the traffic was busy we got at 1845. It began when 2 Bullbuilders explained the process, there were 7 horse races and 1 pig race. The betting was Tote where you could bet 1 - 5 pounds per/house/race and some races you cloud buy a 2 horse forecast. I put 5 pounds on number 7 for every race and bought 7 forecasts. I spent 16 pounds on food and drink. It finished at 2200 and we got home at 2245.

24/06 I went to the Activities room about 1030 to prepare for the Service Users Forum that's beginning at 1100.Julie the Domestics Manager came at 1100 but nobody else came. At 1110 she left about 1115 the Activities group with 1 Carer. At 1130 I started the meeting. Apologies from Scott the Manager and Colin from Catering. The lasted for 30 minutes with NOB and the Next Meeting on 29/07 at 1100. After lunch I went to see Scott and Emma in his office to discuss my Complaints. Scott informed me that new manager Rachel was joining Exemplar on 02/07 with an Induction first and come here 09/07. After the meeting I went to room, put my feet up and used my PC. After tea I used PC.
25/06 I used my PC after breakfast. After lunch I put my feet and watched the TV but at 1600 my bowels opened and carers changed my pad. After tea I used my PC.
26/06 I used my PC before I went to watch Bradford v Halifax ko 1500. Tony my driver/carer from Totalcare came for me at 1130 and we arrived at the Provident stadium at 1215. We went into the Conference and Banqueting Centre and were allocated table 2. We went to table 2 and I asked Tony to get a pint of Soda water. He had half of Coke which cost me 3.50 pounds. At 1320 Robbie Hunter-Paul invited my table to collect our main course, it was Chicken, mashed potatoes and mixed veg with gravy but the head of catering gets mine. After the main course the speaker is introduced which was Tom Orbison a Bradford Bulls player. We had a trio of Chocolate for our dessert. After Robbie held a quiz. When it was finished we went outside to see the match. The final score was 24 - 32 with 5232 attending but the Bulls had 10 players were injured. We went back inside to see the MOM and Rohan Smith give his report but it was too nosey. We left and got home at 1745. Later I had a Chilli 11' Pizza and a portion of Chocolate Fudge cake.
27/06 I used my PC and in my post I received a letter from West Police saying they are going to charge Tony, my driver, for speeding on 19/06 on the Inner Ring road because his speed was 51 mph. I emailed Totalcare to give his details. I asked the nurse if I can have my Care Plan and I got all my coins to make list. After lunch I put my feet and used my PC.
28/06 I tidied my safe this morning and I found my tickets for Leigh which were missing. I used my PC. After Brunch I put my feet up and used my PC. After tea I used my PC.
30/06 I used my PC but cursor button was faulty. I vac my keyboard and it worked again. At 1100 I had 2 Bacon sandwiches because I had an appointment with the Eye Clinic, which I have every year, because of my Diabetes, at 1345. My transport arrived 1415 and we arrived at the Eye Clinic in St. James. We went into the waiting room which was overflowing and nurse called out people that didn't have an appointment to go to the café because there was 120 people in the waiting room with only 80 seats. They move me to the front and a nurse tested my eyes and put some drops in both eyes.

03/05 I used my PC to email Victoria Nunns from Leeds Council to tell her I can't go out to our meeting. At 1230 Victoria Nunns arrived and she told me Leeds City Council can't upgrade the path beween North Sherburn Road ad Hebden Approach through the park because there's no money. I will use the long way around the park. After lunch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.

06/05 I used my PC to find East Leeds Podiatry which is on Osmanthorpe Lane down York Road. We, my driver and me set off at 1230 and arrived at 1250. The Podiatrist called me in at 1300 and she cut my toenails but she cut the skin on one toe. She'd finished at 1310 and we got home at 1345. After lunch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
09/05 I used my PC in the morning, I didn't use my PC over the weekend because I had to stay in bed because my electric wheelchair was broken. In the afternoon I put my legs up and used my PC. After tea I used PC and cleaned my teeth.
10/05 In the morning I used my PC. I had a Bacon sandwich at 1200 because I had an appointment at Pindersfields at 1400 to have my Baclofen pump filled. My transport came at 1215 and arrived at Pinderfields at 1300. We were called at 1330 and at 1405 we went to the waiting room to wait for our transport. It came at 1600 and got home at 1645. After tea I used my PC.
11/05 I got up at 0630 and got washed in bed. I got dressed in my bed and cleaned my teeth. I went to the dining room and had my breakfast. At 0845 my transport came to take me to Chapel Allerton hospital where I had an appointment with a Dermatologist at 0940. He looked at my mole and he told me is was benign. He asked another doctor to confirm it was benign. We got home at 1130. After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.
12/05 I used my PC and the nurse told me we're having my Care plan 3 monthly Review. I asked her for a copy of the minutes. She gave me copy and I added some action points. After lunch I used my PC, I put my feet and I watched TV. At 1600 my bowels opened and carers changed my pad. After tea I used my PC.
13/05 I used my PC before I went to Care Plan Review 3 monthly. After I went to the Coffee morning. After l felt sick and went to bed. I had my tea in bed and after I feel asleep. I woke up at 2200 and asked the nurse if I might have my night time medicine. He gave me it and I said I couldn't breathe. He took relevant tests and decided to phone 999. I was taken to St James to the A and E where I saw a doctor and had a X-Ray on my lungs. I was told I had an infection on my lungs and I was moved on to Ward 29 bed 4 the older persons ward. In Ward 29 there is 1 TV but the patient all sleep all the time. There is no facility for TVs for your bed. I said I only eat Halel food. Also when I got my PC I could access the Hospedia Hospital Wi-Fi. Before on Saturday and Sunday I read my book. On Monday a carer brought my PC. On Tuesday I was discharged and got home at 1800. I had a pizza delivered. I spend the night in a decent place.

20/05 I used my PC before going to the Coffee Morning. After Coffee morning and lunch I put my feet and watched TV. At 1600 I told my carers bowels had opened. They changed my pad and I got ready to watch Sheffield v Bradford at The Oasis Academy Don Valley Leeds Road, Sheffield, S9 3TY. After tea we set off there wasn't a lot of traffic but we turn off at jcn 34 and we headed toward Sheffield on the wrong road. We eventually found the ground at 1915 with a kick off at 1945. There was little facilities. Wheelchair users were parked down the side on a path which ran up to the dressing rooms. At one end most of the supporters sat but they were covered. In was very close at half time it was 10 - 6 but at full time Bradford won 14 - 25 and crowd was 995. We got home 2300 because the M1 was closed near Leeds.
21/05 I used my PC and checked there were other Care Services in North Leeds. In the afternoon I put my feet up and watched TV. After tea I used my PC and spoke to my youngest son in Tenerife via Facebook.
22/05 I went to Tesco's to get my new reading glasses. After Sunday lunch I put my feet up and watched TV. After tea I used my PC.
23/05 I returned my DVD player to Ebay and used my PC. I put 3 requests to Activities, to go to the Race night on 16/06 at Bradford Conference and Banqueting Centre, to go to Bridlington on weekday and to go to Pontefract Racing on 19/06 at 1300. They told me I couldn't go to Pontefract but I could go Bridlington and the race night. After lunch I went out in the garden but it was cold and raining. I came in put my feet up and watched TV. After tea I cleaned my teeth and used my PC.

After lunch I used my PC and at 1400 I went to our lounge to listen singer called Nigel play his guitar and sing for a hour. I came back to my room and use my PC. After tea I used my PC.
25/05 I spent most of my time trying to get someone to take me to Blackpool on Saturday but as yet I havn't succeed. After I used my PC. After lunch I used my PC and looked for a driver to take me to Blackpool on Saturday. I asked Stacey, a carer who works in Ravensdale and she will tell me later. .1615 my bowels opened and my carers changed my pad. After tea I used my PC .
26/05 I spent most of the morning finding out if a carer can take me to Blackpool this Saturday. James will take me if there's no emergencies. In the afternoon I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.

I sent an email to ask the Bradford Bulls asking why they took 13 pounds on 27/06. This morning I used my PC. After lunch I asked the catering manager if I could have a Salmon fillet, Lamb's liver, sirloin steak cooked medium to rare with battered onions rings every Saturday. I went to my room, used my PC, Social Care Direct phoned to say they were investigating and they would get my Social Worker to phone tomorrow.I put my feet up and watched TV. My Life being Disabled April 2016
01/04 I used my PC in the morning. After lunch I put my feet up and watched a movie. At 1530 my eldest son came and we watched sport on TV. After tea I used my PC.
02/04 I used my PC to send an email to the manager and the Operations Manager because the driver is leaving and I want to know what's happening with my Rugby League visits. I got an email from the Chairman of Halifax thanking me for writing my report to the RFL. After lunch I put my feet up, used my PC and watched TV. After tea I used my PC.
03/04 I used PC and got ready to go to Odsal. The Totalcare driver/carer came 1140 and we . set off to Odsal We arrived at 1230 and my driver drop me off at the Conference and Banqueting Centre. I went in and waited for her. The doorman told we were sat on Table 1 and we order 2 drinks. The room filled up and at 1300 the CEO began MCing. At 1330 our meal was served it was Gammon, new potatoes and gardens peas followed by Treacle sponge and Custard. After the main course the speaker Nobby an old coach gave speech about being a coach in Rugby League. Before kick off we had a quiz. At half time we had cup of coffee and full time score was 28 - 46. We had the MM and the coaches thoughts. We got home 1830 and had a Chilli pizza.
04/04 I used my PC to complain about staff shortages over the weekend and I asked London Broncos what time does Hospitality start. Activities asked about my email that I sent to Mark because they are unable to cover Sheffield v Bradford on 20/05 at 1945. They asked me if I could contact Totalcare to see if they would take me to London to see Broncos v Bradford on 01/05 at 1500. I emailed Totalcare. After lunch I went to Tesco's to pick up my glasses. After tea I used my PC and my bowels opened with my carers changed my pad.
05/04 I used my PC in the morning preparing for the meeting with the manager on Friday at 1400. In the afternoon I put my feet up and watched my TV. At 1600 my bowels opened and my carers changed my pad. After tea I cleaned my teeth and used my PC.
06/04 I used my PC before lunch. After I used my PC, put my feet up and watched a movie. After tea I finished watching the movie before I cleaned my teeth.
07/04 In the morning I used my PC. After lunch I used my PC getting ready for tomorrow's meeting with the manager at 1400. At 1400 I went to our TV room where we had a male singer called Brandon . He sang for a hour and after I went to my room. I tried to get my printer working to print off the emails I sent to Mark but it wouldn't. After tea I used my PC and cleaned my teeth.
08/04 I used my PC and I didn't go to the Coffee morning because Activities had cancelled it. After lunch I sorted my items for the meeting with Mark at 1400. We held it in Mark's office and were joined with Nursing Manager and the Activities Manager. Items we discussed were documented by Mark who said I'll get a copy. After meeting I went to my room and used my PC. After tea I used my PC and I cleaned my teeth.
09/04 I used my PC before lunch. After lunch I went out to post a letter. I came to my room, put my legs up, used my PC and watched TV. After tea I cleaned my teeth and used my PC.
10/04 I used my PC until 1200 when I started getting ready to go to Dewsbury to watch Dewsbury v Bradford at 1500. My son came at 1315 and we arrived at Dewsbury at 1400. I paid but steward told us we couldn't go to the Disabled Viewing Platform because the lift was faulty. We went to the side of pitch in front of the North Stand where my son sat but I parked my wheelchair in front of the seats. The started at 1500 and at half time it was 10 - 18 and at full time it was 18 - 52. After the match we went to The Mermaid for Fish, Chips and Mushy Peas followed by a cup of Tea. I home at 1830 and put legs up.
11/04 This morning I used my PC and started tidying my top drawer before lunch. After lunch I continued until tea. After tea I used my PC but my bowels opened and my carers changed my pad.
12/04 I used my PC but the nurse came at 1130 to tell me it was my quarterly Care Plan review with my advocate. After the meeting I used my PC. After Brunch I used my PC and put my feet up to watched TV. After tea I used my PC.
13/04 I used my PC and loaded Windows 10 on my PC in the morning. After lunch I used my PC, put my legs and watched TV. After tea I used my PC.

15/04 In the morning I went to the Coffee morning where we had 2 quizzes. I saw my GP because I had a mole. He reffered me to Dermatologist. After lunch I use my PC, put my feet up and watched TV. After tea my bowels opened and my carers changed my pad. I used my PC.
16/04 I used my PC before lunch. At 1130 my younger brother came to see me. He left when I went for lunch. After lunch I used my PC but I decided to restart because it was running slow. After tea I cleaned my teeth and used my PC.
17/04 In the morning I used my PC. After Sunday dinner I used my PC, put my legs up and watched TV. After tea I used my PC.
18/04 I used my PC, I received a letter from West Yorkshire Police regarding a complaint I made last October about the police when they were guarding a man in St James hospital. The investigation found nothing wrong with their behaviour. I got 3*5 pounds gold coins celebrating the Queens 90th Birthday. After lunch I used my PC, put legs up and watched TV. After tea I cleaned my teeth and used my PC.
19/04 I used my PC and sorted my financial contributions for 2016/7. After lunch I went to Tesco's shopping. When I got home I unpacked my shopping and my bowels opened. My carers changed my pad and I used my PC. After tea I cleaned my teeth and used my PC.
20/04 In the morning I filled in my postal vote and posted back. I used my PC before lunch. After I used my PC, put my feet up and watched TV. At 1600 my bowels opened and my carers changed my pad. After tea I used my PC.
21/04 I distributed the agenda for tomorrow's Service Users Forum. I used my PC and let James the new carer try my car. I ordered a new Bradford Bulls shirt. After lunch I used my PC and at 1530 I went to a meeting with Leeds City Council about the footpath between North Sherburn Road and Hebden Approach but nobody turned up. After tea I used my PC.
22/04 I used my PC and went to the Service Users Forum at 1130. After lunch I put my legs up and watched TV. After tea I cleaned my teeth and watched TV.
23/04 The nurse didn't do my bladder until 0915 and the carers wouldn't do my bed wash until the nurse did my bladder which was 1000. I had a bed bath and got dressed. When I was hoisted into my wheelchair I cleaned my teeth. After breakfast I put feet up and used my PC. After lunch I used my PC, put my feet up and watched TV. At 1600 my bowels opened and my carers changed my pad. After tea I used.my PC.
24/04 After having my breakfast I got ready to go to Odsal and watch Bradford v Worthington. We set at 1115 because wanted some screen wash. We went into Seacroft to the 90p shop were I bought 2 bottles. We got to Odsal 1215 and we went into the Conference and Banqueting Centre and we were informed our table was 2. We had a drink before our meal which was served at 1310.Home made Cottage Pie and carrots. After the main course we had a 20 questions quiz. The dessert was New York style baked Cheesecake served with fresh fruit compote. At 1450 we were to take our seats but we stayed inside to watch the match from viewing gallery. At first it was very close but final score was 52 - 16. There were only 3467 people there.
25/04 I didn't get up until 1100 because I didn't feel good. The nurse told me he's phoned my GP. After breakfast I used my PC. My GP came in prescribed some antibiotics. After lunch my bowels opened and my carers changed pad. I put my feet up and watched my TV. After tea I used my PC.
26/04 I used my PC and read my confirmation from RL to go watch England v New Zealand on 29/10 at John Smith Stadium Huddersfield at 1430, England v Scotland 05/11at 1730 in the Rioch Stadium Coventry, England v Australia on 13/11 at 1430 in the Olympic Stadium London and final on 20/11 at 1430 in Ainfield Stadium Liverpool. After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.
27/04 I used my PC in the morning. In the afternoon I used my PC, put my feet up and watched TV. I got ready to go to Leeds Arena to see Noel Gallagher and the High Flying Birds. My eldest came at 1745 and we set off at 1800 but the traffic was very busy and we didn't arrive at Merrion Centre Car Park until 1900. We went to the Leeds Arena and got in our places at 1930. It was very noisey and the opening band played for 45 minutes. Noel Gallangher started 2100 but by 2145 I asked my son to leave because I felt it. We got home 2230.
28/04 I woke up at 0500 unable to breathe. I told the nurse who called for an ambulance to take to St James. We got there at 0630 and went A and E. At 0800 my carer told me she was going but another carer was on her way. She arrived at 0805 and stayed with me until 0830 because Ravensdale told her I would OK because I can talk. At 1030 I was admitted into J27 bed 5. I stayed there until Saturday evening 30/04. My only enjoyment was my TV EXPIDEA.

My Life being Disabled March 2016
01/03 Oldham RL phoned to ask me if I would pay 70 pounds into their bank account. I bought a red bomber jacket from Amazon. I used my PC. After lunch I asked the nurse if I could go to the Leeds Citizens Panel on the 16/03 from 1600 at Swarcliffe Community Centre and I asked her if I could go to Aldi. When the driver came back we went to Aldi and Asda. When we got home it was 1630. After tea we unpacked my shopping. I used my PC.
02/03 I used my PC and I joined the Bulls big lottery. After lunch I was surprised to see that The Beatles are performing at 1400. I used my PC and went to our lounge to see The Beatles which was a lady singing Beatles song for 1 hour from Cats Pyjamas. After tea I used my PC and loaded Windows 10.
03/03 I used my PC and I advertised for a driver, companion and carer on the staff noticeboard. I paid 850 pounds for 2 season tickets to watch Bradford Bulls in the Conference and Banqueting Centre. After lunch I used my PC and my bowels opened and my carers changed my pad. After trying my son and my wife to go and watch Dewsbury v Bradford in the Challenge Cup I got Wendi at Totalcare to go with me. I changed my PC back to Windows 8 because I didn't like Window 10. After tea I used my PC.
04/03 I phoned Dewsbury to see if they are selling tickets for The Challenge Cup but they will sell them on Monday. I used my PC but 1120 I went to the Activities room for Friday's Coffee morning. After coffee and a big slice of cake we 2 quizzes. After lunch I used my PC but at 1430 my bowels opened and my carers changed my pad. After I used my PC and I put my feet up.
05/03 Last night I wasn't put to bed until 1215 because there was only 1 carer and no nurse on our until Bluebell. The nurse told me she's spoken to my GP who has prescribed a different antibiotic. I used my PC in the morning. In the afternoon I used my PC and put m my feet up. After tea I cleaned my teeth and use my PC.
06/03 This morning after breakfast I got ready to go to Oldham to watch the match Oldham v Bradford. At 1030 somebody from Oldham rang me to tell me the match was postponed due to the weather. I got changed and used my PC before lunch. After lunch I found my new Bradford Away Shirt on the floor. I sent an email to the manager and to the Operations manager complaining about the lunch and my new Bradford Away shirt. I put my feet and used my PC but at 1600 my bowels opened and my carers changed my pad. After tea I cleaned my teeth and used my PC.
07/03 This morning I phoned Dewsbury Rams and ordered a wheelchair space and carer for the Challenge Cup on the 18/03 2000. I switched my PC on but Windows updating. After I replied to Goodman's about my Soundbar. After I used my PC,
08/03 I used my PC in the morning. In the afternoon. I used my PC, put my feet up and watched a movie on my PC. At 1600 my bowels opened and my carers changed my pad. After tea I cleaned my teeth and used my PC.
09/03 I went to Tesco's to pick up my Soundbar which I bought from TescoDirect. I came back and used my PC. After lunch I phoned Amazon to return my old Goodmans Soundbar which is going tomorrow. I put my feet up and used my PC. After tea I used my PC.
10/03 I had my Catheter changed in the morning. I got my parcel ready to send back to Amazon via DPD who are coming to collect between 1457 - 1557. I used my PC and put my feet up. After tea I used my PC.
11/03 This morning DPD picked my old Soundbar for Amazon, I used my PC and I went to the Coffee. After lunch I put my feet up and used my PC. I phoned Halifax and ordered 2 Hospitality tickets for Easter Monday at 1500 it begins at 1300. I put my feet up and used my PC. After tea I used my PC.
12/03 In the morning I used my PC, I wrote a fixture list for Bradford Bulls and I tidier my small drawer. After lunch I put my feet up and watched TV. After tea I used my PC.
13/03 I got ready to go to Batley where Bradford are playing. I put on my thermal underwear even though it's sunny and warm. My eldest son is taking me at 1300. I used my PC until 1230 where I watched TV. My son came 1315 we got Batley at 1400. Andrew paid 17 pounds for him and we parked next to the Disabled entrance. We went in and the was shining onto new stand and we decided to go to the old stand where there was a Disabled Viewing Platform. Wheelchairs sat in front our carers but there wasn't much room and when match started Andrew moved to sit in the seat above. At half time the score was 12 - 14 but the final score was 24 - 24 with Batley getting a penalty on 80 minutes. We left soon after but crowd about 2,500 it took us 30 minutes to get out Batley's car parks'. We got home at 1800 and ordered a Chilli Pizza.

15/03 I opened a parcel from Bradford Bulls it was my order from the shop, a scarf, a woolly hat, a pair of gloves, a car air fresher, two holders for my membership cards, a car sticker and drink dispenser. I used my PC, put my feet up and watched TV. After tea I used my PC but I tried to load some fixes but it didn't load and switched off.

18/03 I got an invitation for me and 3 guests to attend a presentation at Ferham House( HQ ) on 25/04 at 1030 to presentation my Asdan certificate. I used my PC in the morning and watched a movie. After lunch I put my feet up and used my PC. At 1530 my bowels opened my carers changed my pad, put my thermal vest and long john on. The put my Bradford Bulls away shirt on and trousers on. They got me ready to go to Dewsbury for the match which kicks off at 2000. After tea I waited for the driver/carer to take to Dewsbury She came with another carer who showed her how to fasten my wheelchair into my car. We set off 1815 and arrived at Dewsbury's ground at 1900 because there wasn't any traffic jams on the motorways. We were directed to the Disabled Parking which was outside the main entrance. We entered an stewards asked us for our tickets and showed us a Disabled lift. I could get in but carer had to use the stairs. I got out on level 2 meeting my carer and we followed the sign for Disabled viewing. We entered and we were the only people in. Next door was the press. About 6 wheelchairs and 6 carers could fit in. My carer wet for me a cup of Coffee from the bar downstairs but they don't sell Coffee/Tea. She went outside to Burger shop. I could most of the pitch except the steel girders holding up the roof. The match started and at half time it was 14 - 12 but after thrilling second half Dewsbury won 31 - 30. We got home at 2230.
19/03 I asked the nurse if she would book me for eye test and could I go to Castleford cooking. She gave me an invitation to attend Ferham House( HQ ) on 25/04 at 1030 to receive my ASDAN certificate. I used my PC. After lunch I used my PC, put my legs up and watched TV. At 1600 my bowels opened and my carers changed m pad. After tea I used my PC.
20/03 I called a carer to get me up BUT she said I can't get up because she's by herself until another carer come in I'll just have wait. I fell asleep was woken by another carer at 1000 who told me there's nobody in kitchen but Terri one of carers from upstairs has volunteered to do Sunday lunch. The carer got me washed dressed and I had breakfast. After breakfast I used my PC. After lunch I put my feet up and watched TV. After tea I used my PC.
21/03 Again shortage of kitchen and carers plus no management. I used my PC to email Helen and Amanda to escalate. I got email from Dewsbury telling me they had sold out of Hospitality for the Bradford match on 10/04. In the afternoon I put my feet up and used my PC. After tea I used my PC.
22/03 I used my PC in the morning. In the afternoon I used my PC, put my feet up and watched TV. At 1600 I some shopping ordered from Asda to be delivered tomorrow at 1200 - 1400. After tea I used my PC.

My driver was Ben and we set off at 1615. We got to Odsal at 1755 with our meal planned to start at 1800. It started at 1830 because everybody was late due to heavy traffic. We had Chicken and Leek pie with new potatoes, carrots and onion gravy followed by Strawberry Cheesecake with cream. We had a 20 questions quiz hosted by Robbie Hunter-Paul. My carer and I decided to stay inside.
At half time the score was 10 - 12 and at full time it was 28 - 20. After match we sat at our table to listen to 2 guys playing guitars and singing. About 1015 Jimmy Lowes gave his thoughts on the match, After we went home.
25/03 I went to the Coffee morning. After cake and coffee we had 4 quizzes. After lunch I put my feet up and used my PC. After tea I used my PC.
26/03 I loaded Windows 10 on my PC. After lunch I put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
27/03 I used my PC with Windows 10 but I decided to changed back to Window 8 because I didn't like it. In the afternoon I used my PC, put my feet up and watched TV. After tea I used my PC.
28/03 In morning I got ready to Halifax to watch Bradford at 1500. We set off at 1100 and arrived at The Shay at 1200 with our meal in the Executive Club being served at 1245. In the room which holds 300 was full by 1230. There was a comedian as MC. The meal consisted of Chicken liver pate with pink peppercorn butter, chilli jam and melba toaster as a starter. Main course was Traditional roast beef and Yorkshire pudding served with seasonal potatoes and vegetables. Followed by Baileys cheesecake with cream. At half time we were served tea and coffee.
At 1450 we were directed to the Disabled Wheelchair Viewing Platform which was outside the dining room. It was an excellent view with 10 wheelchair spaces and 10 seats. There was plenty of room behind. The score at half time was 10 - 0 and at 14 - 10. We went back into the dining room and had a cup of coffee. The MC announced the MOM and their coach gave his thoughts. We set off at 1800 and got home at 1900. I ordered a 11' Chilli Pizza from Sopranos.
29/03 I used my PC, sent email to London Broncos asking about their Hospitality and ordered tickets for the Summer Bash Saturday and email Genealogy Jersey to stop their yearly payment. I got my glasses and my HC2 ready for my appointment at Tesco's Opticians at 1450. I set off at 1400 by myself and arrived at Tesco's at 1430. I went in the pound shops first. I went Tesco's Opticians where he tested my eyes. He told me there was little difference between my eyes 2 years ago and today. He said with my HC2 I can get both sets of glasses free. He would recommend getting new glasses because my glasses are scratch. I chose new frames to darken with the sun which cost me 64 pounds. I through Tesco's to get some shopping. On the home I skidded on the path through the park. After I went very slowly to get home. After I used my PC.
30/03 In the morning I used my PC. After lunch I used my PC. At 1430 I went to the manager's office to see a cook whose interviewing for the cook's job. After I went my room, put feet up and used my PC. After tea I cleaned my teeth and used my PC.
31/03 I used my PC to book an appointment with East Leeds Podiatry for 06/05/16 at 1300. I phoned London Broncos and 2 Hospitality tickets for 01/05. I sent an email to the manager about my contribution for my fees to Ravensdale in 15/16 why they increased in July. He told me Wakefield MDC had reduced their contribution because my pension had gone up. I sent an email Social Care Direct for assistance. After lunch I put my feet up and used my PC. After tea I used my PC to change my passwords.
02/03 I used my PC and I joined the Bulls big lottery. After lunch I was surprised to see that The Beatles are performing at 1400. I used my PC and went to our lounge to see The Beatles which was a lady singing Beatles song for 1 hour from Cats Pyjamas. After tea I used my PC and loaded Windows 10.
03/03 I used my PC and I advertised for a driver, companion and carer on the staff noticeboard. I paid 850 pounds for 2 season tickets to watch Bradford Bulls in the Conference and Banqueting Centre. After lunch I used my PC and my bowels opened and my carers changed my pad. After trying my son and my wife to go and watch Dewsbury v Bradford in the Challenge Cup I got Wendi at Totalcare to go with me. I changed my PC back to Windows 8 because I didn't like Window 10. After tea I used my PC.
04/03 I phoned Dewsbury to see if they are selling tickets for The Challenge Cup but they will sell them on Monday. I used my PC but 1120 I went to the Activities room for Friday's Coffee morning. After coffee and a big slice of cake we 2 quizzes. After lunch I used my PC but at 1430 my bowels opened and my carers changed my pad. After I used my PC and I put my feet up.
05/03 Last night I wasn't put to bed until 1215 because there was only 1 carer and no nurse on our until Bluebell. The nurse told me she's spoken to my GP who has prescribed a different antibiotic. I used my PC in the morning. In the afternoon I used my PC and put m my feet up. After tea I cleaned my teeth and use my PC.
06/03 This morning after breakfast I got ready to go to Oldham to watch the match Oldham v Bradford. At 1030 somebody from Oldham rang me to tell me the match was postponed due to the weather. I got changed and used my PC before lunch. After lunch I found my new Bradford Away Shirt on the floor. I sent an email to the manager and to the Operations manager complaining about the lunch and my new Bradford Away shirt. I put my feet and used my PC but at 1600 my bowels opened and my carers changed my pad. After tea I cleaned my teeth and used my PC.
07/03 This morning I phoned Dewsbury Rams and ordered a wheelchair space and carer for the Challenge Cup on the 18/03 2000. I switched my PC on but Windows updating. After I replied to Goodman's about my Soundbar. After I used my PC,
08/03 I used my PC in the morning. In the afternoon. I used my PC, put my feet up and watched a movie on my PC. At 1600 my bowels opened and my carers changed my pad. After tea I cleaned my teeth and used my PC.
09/03 I went to Tesco's to pick up my Soundbar which I bought from TescoDirect. I came back and used my PC. After lunch I phoned Amazon to return my old Goodmans Soundbar which is going tomorrow. I put my feet up and used my PC. After tea I used my PC.
10/03 I had my Catheter changed in the morning. I got my parcel ready to send back to Amazon via DPD who are coming to collect between 1457 - 1557. I used my PC and put my feet up. After tea I used my PC.
11/03 This morning DPD picked my old Soundbar for Amazon, I used my PC and I went to the Coffee. After lunch I put my feet up and used my PC. I phoned Halifax and ordered 2 Hospitality tickets for Easter Monday at 1500 it begins at 1300. I put my feet up and used my PC. After tea I used my PC.

13/03 I got ready to go to Batley where Bradford are playing. I put on my thermal underwear even though it's sunny and warm. My eldest son is taking me at 1300. I used my PC until 1230 where I watched TV. My son came 1315 we got Batley at 1400. Andrew paid 17 pounds for him and we parked next to the Disabled entrance. We went in and the was shining onto new stand and we decided to go to the old stand where there was a Disabled Viewing Platform. Wheelchairs sat in front our carers but there wasn't much room and when match started Andrew moved to sit in the seat above. At half time the score was 12 - 14 but the final score was 24 - 24 with Batley getting a penalty on 80 minutes. We left soon after but crowd about 2,500 it took us 30 minutes to get out Batley's car parks'. We got home at 1800 and ordered a Chilli Pizza.

16/03 I started to sort it out but I had to reload my PC. In the afternoon I put my feet up and watched TV until 1545 when bowels opened and my carers changed my pad. After tea I used my PC.

18/03 I got an invitation for me and 3 guests to attend a presentation at Ferham House( HQ ) on 25/04 at 1030 to presentation my Asdan certificate. I used my PC in the morning and watched a movie. After lunch I put my feet up and used my PC. At 1530 my bowels opened my carers changed my pad, put my thermal vest and long john on. The put my Bradford Bulls away shirt on and trousers on. They got me ready to go to Dewsbury for the match which kicks off at 2000. After tea I waited for the driver/carer to take to Dewsbury She came with another carer who showed her how to fasten my wheelchair into my car. We set off 1815 and arrived at Dewsbury's ground at 1900 because there wasn't any traffic jams on the motorways. We were directed to the Disabled Parking which was outside the main entrance. We entered an stewards asked us for our tickets and showed us a Disabled lift. I could get in but carer had to use the stairs. I got out on level 2 meeting my carer and we followed the sign for Disabled viewing. We entered and we were the only people in. Next door was the press. About 6 wheelchairs and 6 carers could fit in. My carer wet for me a cup of Coffee from the bar downstairs but they don't sell Coffee/Tea. She went outside to Burger shop. I could most of the pitch except the steel girders holding up the roof. The match started and at half time it was 14 - 12 but after thrilling second half Dewsbury won 31 - 30. We got home at 2230.
19/03 I asked the nurse if she would book me for eye test and could I go to Castleford cooking. She gave me an invitation to attend Ferham House( HQ ) on 25/04 at 1030 to receive my ASDAN certificate. I used my PC. After lunch I used my PC, put my legs up and watched TV. At 1600 my bowels opened and my carers changed m pad. After tea I used my PC.
20/03 I called a carer to get me up BUT she said I can't get up because she's by herself until another carer come in I'll just have wait. I fell asleep was woken by another carer at 1000 who told me there's nobody in kitchen but Terri one of carers from upstairs has volunteered to do Sunday lunch. The carer got me washed dressed and I had breakfast. After breakfast I used my PC. After lunch I put my feet up and watched TV. After tea I used my PC.

After I emailed my eldest son to asked him would he take to O2 shop on Wednesday afternoon to get new Alcatel 2045X.
05/01 Crossgates O2 phoned to tell me they can't order an Alcatel 20.45X. I emailed my eldest son to asked him if he could order one. After lunch I used my PC, put my feet up and watched a movie. After tea I cleaned my teeth and used my PC.

During lunch my GP came look at my Cirrhosis that's developed on my head. He prescribed some shampoo for my head. After I used my PC, put my feet up and watched a movie. After tea I cleaned my teeth and I used my PC.
16/01 I didn't get up all day.
17/01 I used my PC and sent an email to Totalcare telling them the procedure to use and the matches I want go to, 9 in total. After Sunday lunch I used my PC , I put my feet up and watched TV. After tea I used my PC.
18/01 I phoned Leeds Rhinos because they had sent me tickets, a wheelchair space and a carer to my old residence Dearnevale. I told them my new address and mobile details and they told me that Disabled parking tickets will be sent out this week. I phoned the HC1 enquiry line to request a form because mine ran out yesterday. I used my PC. After lunch I used my PC and told TotalCare of my new mobile. After I put my legs up and watched TV. After I cleaned my teeth and used my PC. I got a reply from TotalCare saying my proposals are acceptable.
19/01 I ordered 2 tickets for the Bradford v Featherstone on 07/02 at 1500 in the Conference and Banqueting Centre. I used my PC. After lunch I used my PC, put my feet up and watched. At 1600 my bowels opened and my carers changed my pad. I used my PC. After tea I used my PC and uninstalled my frostwire.

22/01 I stayed all day because I didn't want to get up. My GP came because I was feeling low and said she will review my medicine.
23/01 At 0830 I told the senior carer that I was ready for my bladder wash, At 0930 I told a carer that I hadn't had my bladder wash and could she tell the nurse. The nurse came and she said this is first time you've requested for your bladder wash. After my bladder wash I tried to get into shower room but I didn't have shower sling. I got dressed, cleaned my teeth and got shaved. The Cirrhosis on my head had got worse and my nurse called my GP. After breakfast I used my PC. After lunch I put my feet up and watched TV. At 1500 my GP came to look at my Cirrhosis and gave a prescription for some cream to apply on my face and some antibiotics. After tea I used my PC..
24/01 I used my PC waiting for the nurse to clean face and apply cream. The carer who was due to take me to watch Leeds v Bradford has phoned to tell he's not coming in today because he's got a family bereavement. After lunch I used my PC, put my feet up and watched TV Tea was not served until 1745 and although I asked for Corn Beef Hash I got a plain Omelette with baked beans. After tea I used my PC and sent an email to manager complaining about my tea.
25/01 In the morning I used my computer and I read that the fixture time for Bradford v London is changed from 1500 to 2000 on 25/03. I emailed TotalCare to tell them I won't be going. After lunch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
26/01 I phoned my eldest son because it's his 30th Birthday. I used my PC and phoned the Bulls to find out if the match on Sunday has been rescheduled to Friday. I spoke to Laura the manager and she told me to ring her Friday. She told me that tickets for Bradford v Featherstone will be posted out tomorrow. After lunch I phoned the customer helpline regarding my HC1 form which I asked for last Tuesday but I havn't got. They are going to send me another HC1 form. I put my feet and watched TV. 23/01 At 0830 I told the senior carer that I was ready for my bladder wash, At 0930 I told a carer that I hadn't had my bladder wash and could she tell the nurse. The nurse came and she said this is first time you've requested for your bladder wash. After my bladder wash I tried to get into shower room but I didn't have shower sling. I got dressed, cleaned my teeth and got shaved. The Cirrhosis on my head had got worse and my nurse called my GP. After breakfast I used my PC. After lunch I put my feet up and watched TV. At 1500 my GP came to look at my Cirrhosis and gave a prescription for some cream to apply on my face and some antibiotics. After tea I used my PC..

The carer who was due to take me to watch Leeds v Bradford has phoned to tell he's not coming in today because he's got a family bereavement. At 1500 we set off in my car and we got there 1540 we were 10 minutes late. They gave the next slot at 1600. She cut my toenails and confirmed my next appointment I'll phone on the 28/03. After tea I used my PC and cleaned my teeth.
02/02 In the morning I used my PC and complained to Asda because they sent a Chicken Tandoori Masala instead of a Chicken Jalfrezi and I loaded Windows 8.1 because I didn't like Windows 10. After lunch I used my PC until 1345 when my transport came to take me to Pinderfields hospital in Wakefield to have my Baclofin pump filled up. We arrived there at 1420 and went to the Doctors room where a nurse put me on to a bed. The doctor filled my Baclofin pump up and increase it. We came home and arrived at 1600. 04/12 This morning I used my PC and I went to the Coffee morning where we had 2 quizzes. After lunch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.

06/12 I used my PC in the morning. In the afternoon I used my PC, put my legs up and watched TV. At 1600 my bowels opened and carers changed my pad plus my trousers. After I cleaned my teeth and used my PC.
07/12 I got up at 0745 but I should have got up 0700 because I had Dentist appointment at 1100 in the Reginald Centre in Chapel Allerton. After breakfast my transport came at 1000 and we arrived at 1030. I saw the dentist who gave me a filling and sprayed my teeth to protect them. I got home at 1230. After lu I used my PCnch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
08/12 After breakfast I used my PC until 1200 when I had my lunch. After lunch I used my PC until 1345 when my transport came to take me to Pinderfields hospital in Wakefield to have my Baclofin pump filled up. We arrived there at 1420 and went to the Doctors room where a nurse put me on to a bed. The doctor filled my Baclofin pump up and increase it. We came home and arrived at 1600. I used my PC before tea. After tea I used my PC.
09/12 I had my monthly Care Plan review this morning. After I used my PC, put my feet up and watched TV. After I used my PC.
10/12 I used my PC in the morning. I used my PC in the afternoon, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
11/12 I used my PC and was told that the Coffee morning has been postponed until 1500. I emailed Asda because my shower gel Tropical Breeze at 55p for 500ml was change to Relax at 75p for 250ml. After lunch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and I used my PC.

14/12 I used my PC after waiting for my carpet cleaning because it was soaked after I had a shower. After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.

16/12 My eldest son came at 1030 and left at 1130. I used my PC before lunch. After lunch I used my PC, put my legs up and watched TV. I didn't have any tea because I felt sick.
17/12 I used my PC in the morning and got my tickets to see Dynamo at 1930 in the First Direct Arena tonight from my safe. After lunch I used my PC, but at 1400 I felt sick and drowsy. My carers told the nurse and I said I wanted to go to bed. Later the nurse asked me if I felt better but I told him no. He told the carer that I didn't want to go to see Dynamo. I stayed in bed all night but I didn't get much sleep.
18/12 I got a new tube of toothpaste from the nurse and I used my PC in the morning. After lunch I used my PC, put my feet up and watched TV. After tea I used my PC and reset my PC..
19/12 I used my PC and ordered 20 white hankies from Ebay. After lunch I used my PC and at 1430 my bowels opened and my carers changed my pad. I read my new paper book Conqueror by Conn Iggulden. After tea I used my PC.
20/12 In the morning I used my PC and use my PC, put feet up and watched a movie. After tea I used my PC.
21/12 In the morning I used my PC. I used my PC, put my feet and watched TV in the afternoon but I didn't have my tea. I said I want to go to bed.

23/12 I used my PC and I got my hankies from Ebay. After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.
24/12 I used my PC and asked my carers if they would cut my hair. I had my hair cut 1130. After lunch I used my PC, my bowels opened and my carers changed my pad. I put my feet up and watched TV. At 1630 I used my PC. I went to the Xmas party but it was too noisy and I went to my room.
25/12 Both my sons came to see me in the morning. After Xmas dinner I put my feet up and watched TV. I didn't have any tea and I continued watching TV.
26/12 I used my PC and I put in an order to Asda to be delivered on 30/12 at 1300. After lunch I used my PC, put my legs up and watched a movie but at 1600 I told my carers that my bowels had opened. They told me they hadn't. After tea I finished watching a movie.
27/12 I used my PC in the morning. In the afternoon I used my PC, put my feet and watched a movie. After tea I used my PC.
28/12 In the morning I used my PC. In the afternoon I used my PC, put my feet up and watch TV. After tea I cleaned my teeth and used my PC.
29/12 I used my PC in the morning before I went to Seacroft hospital for my chest X-Ray at 1345. My transport at 1315 and we arrived at 1340. We went to the X-Ray dept., waited 10 mins and we went to take my coat and my Bradford Bulls shirt off. They took a X-Ray of my chest for my GP. I got dressed and went back into reception to wait for my transport. The transport came at 1530 and we got home at 1600. I went to my room, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
30/12 I used my PC in the morning and I asked Ben, a carer, if he would take me Leeds v Bradford on 24/01 at 1500 - Yes. After lunch I used my PC and starting to change password. I put my feet up and watched TV. After tea I cleaned my teeth and use my PC.
31/12 In the morning I used my PC to change my passwords.
