I used my PC to order from Amazon, a new mobile Alcatel GSM/20.45X2045. We had an Indian dancer at 1100 for 30 minutes. After lunch I used my PC and my mobile Podiatrist came to check my feet. I waited until 1900 for Lee, my driver/assistant, to come. He didn't come I check my email because my new mobile wasn't working to find out he's stuck at Newcastle. 04/10 Thurs This morning at 0130 I woke up because I was too warm. I called carer to check my heating was off. She read the temperature 25'C coming from the underfloor heating. She couldn't open my window so she put my fan on. I used my PC in the morning. After lunch I used my PC, put my feet up and watched TV. At 1745 I called for the carers to change me because I was going with my sons at 1830 to the Fisherman's Lodge in Street Lane, Moortown. My eldest son phoned to tell he's busy because o2 had been down all afternoon. My youngest son phoned to tell he's running late. He came at 1850 and we arrived at 1920. We both had the Haddock fillet, chips and mushy peas. It was good and I got home at 2045.

Honey Roast Gammon carvery, Yorkshire pudding, roasted new Potatoes and seasonal vegetables(carrots & cabbage). Raspberry & White Chocolate Cheesecake served with Fruit Coulis & Chantilly Cream. First team news, pre match analysis with injured first team players and a quiz. Bradford won 27 - 8 and were promoted to the Championship. When I got home at 1830 I ordered a Chicken Vindaloo and a Chocolate Fudge cake from Soprano's.

I used my PC, until 1500 when the nurse Jess came to take me to Old Trafford to watch Warrington v Wigan in the Super League Grand Final. We arrived 1630 and parked Disabled Car Park W2, very close to the stadium. We had a Cheeseburger £6 and we went to the wheelchair entrance. We went to our places/seat Wheelchair platform Section EL 135 level 2 Seat/place 7 which behind the posts in East stand. The wheelchair platform are about 2.5 metres wide and view is unbelievable. The group performing was The Blossoms from Warrington. The started at 1800 but it was all defence no attack. It was a dour performance from both sides Wigan 12 - 4 with Stefan Ratchford won MOM. After the ceremony it was 2100. We had booked a table at Mughli's for 2100 but Jess was to tired and we went home. I got at 2215.

18/10 Thurs I got my HC2 form out because I've got an appointment with my Dentist at 1455 to have a filling. I used my PC and after an All Day Breakfast at 1200 I had my flu jab. I used my PC and at 1345 my Patient Transport came to take me to me Dentist at The Reginald Centre in Chapel Allerton. We arrived at 1415 and my appointment was for 1455. The Dentist filled my tooth and cleaned my teeth. I arranged another appointment with receptionist for 6 months. We went to the Fushion Reginald café to have a Coffee. The person serving started arguing with a customer. The transport came at 1615 and got home at 1650. I used my PC to order Fillet Haddock and Mushy Peas from Just Eat. They arrived at 1800 and after I used my PC.

27/10 Sat I got ready to go to KCOM Stadium to watch England v New Zealand k.o. 1430. At 1040 my eldest son came and arrived at the KCOM Stadium at 1145. We went up in the lift to level 1 and into The Circle restaurant to be shown our table No.2. The meal was Tomato & Corinder soup, breast of Chicken, Garlic, potatoes, carrots and green beans, ice cream & cheese cake plus coffee. Andy Gregory and Lee Crooks talked and Stephan Latchford. At 1415 we were invited to take our places. We went to the Wheelchair viewing platform which is on the half way line. You could fit 3 wheelchairs and 3 carers but I and my son used it. It s brilliant view. England won 18 - 16. We got home at 1830 and I ordered a 9" Chilli pizza and a Chocolate Fudge cake from Soprano's.

My Life being Disabled Sept 2018
01/09 Sat I used my PC before lunch. At 1330 Lee, my driver, picked up to take me Sharks Park to watch Shaw Cross play York Acorn 14 - 18. After we to The Halfway pub/restaurant. After we come at 1800. I used my PC and I cleaned my teeth.
02/09 Sun I used my PC until my driver Lee came to take to Odsal. We arrived 1240 and went in the shop. I bought a purple hoodie, a purple polo shirt, a black pair of shorts and a Bulls glass. We round to the Southbank Centre, Lee drop me off to park my car. We went the Legends Suite and we were sat at table no. 2. Stuart Duffy was our MC who welcomed us. The main course was Honey roast Gammon, new potatoes and mix of vegetables. The dessert was Chocolate Brownies with cream. At 1455 we went outside to watch the match. At half time it was 20 - 0 and full time it was 54 - 6. I got home at 1800, I ordered a 9" Chilli pizza and Chocolate Fudge cake.

06/09 Thurs I used my PC and emailed Leeds Utd. to find out what they charge in the Peter Lorimer suite. I booked a table for Tuesday at 1930 Salvo's restaurant. After lunch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
07/09 Fri I used my PC and I was told the nurse haven't booked transport to take me to my dentist where I've appointment at 1315. Activities are taking me using my car. We set at 1230 to the Leeds Community Healthcare, Dental service in Chapeltown. We arrived at 1245 but the dentist is closed until 1315. We went in the café and had a cup of Coffee. We went up to level 1 and check in at the reception. The dentist took me in her's room where she examined my bottom teeth. She said 1 of my fillings has broken and she wants to replace it and she examined my false teeth. I went to the reception to make another appointment for 18/10 at 1455. On the way back we stopped at Rockford's fish & chips shop. I got Haddock fillet and a large mushy peas. I ate them when we got home. After I used my PC. After tea my bowels opened and carers changed my pad, shorts plus my shirt. I used my PC.
08/07 Sat I got ready to go to watch Featherstone Lions v Shaw Cross at 1330. I used my PC before I had my lunch. After I used my PC and 1330 my driver came and took to Featherstone Lions home pitch. It was raining and the woman's game had stopped because it was half time. After full time the match started Featherstone Lions were best team and half time the score was 16 - 0. At fulltime the score was the same. After we went to a Toby restaurant near to Asda in Wakefield. I had 4 meats carvery costing less than £7 and Lee had Prawn sandwich. Mine was rubbish and Lee's came with Yorkshire pudding and roast potatoes. I complained to the manager and came without paying. I got home at 1800.

They put a big urine bag on, change my trousers and changed my shirt. I used my PC. At 1900 my driver arrived to take me to Salvo's in Headingley. It was very busy, the manager brought a ramp out because I had to negotiate 4 steps. Firstly from the car onto the road, road onto the footpath, footpath into Salvo's and finally from the bar to the restaurant. I caught my elbow and waitress dress it. I order Kiev pizza which was folded with Chicken and Ham. We finished at 2030 and got home at 2100.

15/09 Sat I used my PC until 1200 when I went to the dining room and had my lunch. After I went to my room and used my PC. At 1330 my driver/assistant came for me to take Shaw Cross who were playing Ince Rose Bridge. We arrived at Shaw Cross at 1430 and the match had kicked off. The final score was 24 - 26. We went to the Wetherby Whaler in Wield for our tea. I got home at 1830.
16/09 Sun I used my PC until 1030 when my driver/assistant took me to the Vestacare stadium in Oldham. It took us 3 hours to get there because the M62 was closed from jcn 26. We used the A62. We had differculty parking until the Safety manager came and we parked inside We went to the Hospitality Sponsors Lounge and we were given Mince beef and potatoes pie with mushy peas followed Lemon cheese cake and a cup of Coffee I told Lee my urine bag was full and we went down a narrow corridor with 2 right angled turns but I couldn't get threw. I asked somebody if there was another disabled toilet they told me in the changing room in another hut. We went in changing rooms and found the disabled toilet. Lee emptied my bag and went t find the safety manager to find out where can see the match. He said there's no where wheelchair user can see the match. Lee and I place next to the path to changing rooms that the stewards don't allow spectators to stand. I could see about 40% of the pitch. Bradford won 16 - 24. We waited until crowd dispersed over 1k and we got home at 1845. I ordered a chicken madras with chocolate fudge cake from Soprenos.

We ordered our drinks only to find they were free because last time we came I complained the beef was tough. We got Roast Loin of Pork, Yorkshire pudding, roast new potatoes, seasonal vegetables, Apple sauce and Sage & Onion seasoning. Dessert was rich chocolate mouse served with Honeycomb Chantily salted Caramel shards. After Robbie Paul interviewed a player, We had a quiz and we were invited to go outside to watch the match. The final score was 52 - 7. When I got home I ordered a 9" Chilli pizza and Chocolate Fudge cake from Soprano's.

23/09 Sun I used my PC until 1200 when my driver/assistant came for me to take me Odsal. We got there at 1240 and it was full in the Legends suite. We were allocated table 2. We ordered our drinks only to find they were free because last time we came I complained the beef was tough. We got Roast Loin of Pork, Yorkshire pudding, roast new potatoes, seasonal vegetables, Apple sauce and Sage & Onion seasoning. Dessert was rich chocolate mouse served with Honeycomb Chantily salted Caramel shards. After Robbie Paul interviewed a player, We had a quiz and we were invited to go outside to watch the match. The final score was 52 - 7. When I got home I ordered a 9" Chilli pizza and Chocolate Fudge cake from Soprano's.

25/09 Tues I went to the Service Users Council, yesterday, at Scotia Heights Stoke. The meeting started at 1230 with lunch. The meeting lasted until 1500 and we got home at 1830. I ordered a large haddock fillet and a large mushy peas from Just Eat.

My Life being Disabled August 2018
01/08 I used my PC, sent a text to LEE about Saturday and left a message to North Wales about Hospitality. After lunch I put my feet up and watched TV. After tea I used my PC.
02/08 I had my monthly Care Plan review for July first thing. After I used my PC in the morning and my GP called because I had stabbing pain in my right leg. After lunch I put my feet up and watched the TV and 1630 I tried to contact the Employment and Support Allowance. After tea I tried to contact the Employment and Support Allowance and use my PC.
03/08 Fri I told the Employment and Support Benefits that I've changed my bank details. I used my PC until at 1100 I went to the breakfast club. After lunch I used my PC to vote for Labour party members standing for xxxx. I put my feet up and watched TV. After tea I used my tea.
04/08 Sat I went to Odsal at 1200 to watch Bradford v Whitehaven. My Hospitality meal was Roast Pork carvery followed Strawberry Cheesecake. Bradford won 46 - 0. When I got home I ordered a Chilli 9" pizza and a Chocolate Fudge cake from Soprano's.

06/08 Mon I used my PC and reported my Wheelchair because the front left wheel has no tread but the front right wheel has a tread. They are coming on Thursday. After lunch my bowels opened and my carers changed my pad. I put my feet up and watched my TV. After tea I used my PC.
07/08 Tues I used my PC and after lunch I used my PC. At 1440 I went to the Activities room to make Cards. At 1615 I went to my room until 1655. After I used my PC.
08/08 Weds I used my PC until 1200 when my eldest son took me to the TSB in The Headrow, Leeds and to the Pendas Arms in Whinmoor Leeds where we had lunch. When I got home I put my feet up and watched TV. After tea I used my PC.
09/08 Thurs I used my PC before lunch. After the wheelchair repair came and changed my front wheels. I used my PC, put my feet up and watched TV. After tea I used my PC.
10/08 Fri I used my PC and at 1030 I went to the Activities room to join the Breakfast club in theme was Continental. I had pieces of Salami and thin Ham with a Croissant also Orange juice. After I used PC. After lunch I used my PC and watched TV. At 1630 I had my monthly Care Plan review. After tea I used my PC.
11/08 Sat I used my PC and after lunch I got ready to go to Shaw Cross who were playing Pilkington. We set off at 1330 and arrived at 1415. The kick off was 1430 and nearby the Summer Fair was going. We parked on the field and went Shaw Cross bench. It was close game with Pilkington winning 16 - 24. After we went to Asda and The Mermaid fish restaurant. We got home at 2000.
12/08 Sun At 1000 my driver came to take me to watch North Wales v Bradford. They play in Wrexham at Queensway stadium. We got there at 1230 and went to the Hospitality tent ! There were 6 tables each with 8 places. I got a Coffee and a pint of lager. We picked an empty table to drink at. I enquired what time does the food get served. The CEO of the North Wales told me it's an outside firm but usually bring it 1330. My bag was full and I asked where is the Disabled toilet. It's under the main stand. 1 toilet for Ladies, 1 toilet for gents and 1 toilet for Disabled. It was dirty and very small. To get there I had to over cut grass and tarmac path about 200 metres. We back to the Hospitality tent and the caters arrived 1345. I told my driver to tell them I need my food ASAP because I'm Diabetic. It arrived after 10 minutes it was Chicken pasta with sauce and Olives with lettuce. It wasn't worth £10. After we found the wheelchair viewing platform at the front of the main stand. It was 2 metres above the 2 running tracks before the Rugby League pitch. The match started at 1430, half time 0 - 20 and full time 0 - 48 with crowd of 886. We waited the crowd dispersed and went to the Disabled toilet under the offices but there was a wheelbarrow. We went inside and found a clean Disabled toilet. After at 1700 we left to go home and got home at 1930. I ordered a chilli, garlic masala from Sopranos.
13/08 Monday At 1030 Service User, 3 carers and a driver for our mini bus left Ravensdale to go to Bridlington. We got there at 1300 and parked next the Spa. We went to Baynham Fish restaurant because they have a ramp. After we went down to South side and had an ice cream it was warm. 2 carers put nuts theirs which the salt on the nuts melted their ice cream. We set off at 1500 and 1715 we got home. After tea I used my PC.
14/08 Tues I used PC but at 1200 the OT came to change my slings for new ones. After lunch I used my PC all afternoon. After tea I used my PC.
15/08 Weds I used my PC in the morning and in the afternoon I used my PC, saw Zoolab at 1515 to 1615, The OT came to sort out my new slings. After tea I cleaned my teeth and used my PC.
16/08 Thurs I got my MOT certificate via the post. JCT 600 at Wakefield took it yesterday. I didn't get up until 0830 because the night staff were busy at 0700. I used my PC until 1200 when the Senior Carer came to cut my hair. After I used my PC all afternoon. After tea I used my PC.
17/08 Fri I used my PC and 1100 I went to Breakfast club with an Indian theme we had Samosas, Onion Bhajis, various curries and naan bread. I left at 1200 to get ready to go Franco's in Thorpe Park. We got to Franco's at 1245 and ordered a Calzone Kiev and Soda water, my carer ordered a 55 orange. It was very busy and my Calzone Kiev came 1325. It was large and left 50% of it. I asked waiter for bag to take it home. I asked for my bill which was £18.30. I gave him £20 and we went outside to wait for my driver. She came at 1400 and we got home at 1430. At 1500 I went to the Activities room for the Food forum. After I used my PC. After tea I used my PC.
18/08 Sat I used my PC to order my Braford Bulls 2019 Hospitality season ticket(£1,022.95) and a Shaw Cross sharks leisure shirt from Ravensports. After my all day breakfast I used my PC. At 1330 my driver/assistant called to take me to Shark park to watch Shaw Cross v Lock Lane, Castleford. We watched all the match next Shaw Cross's dug out but Shaw Cross lost 6 - 58. After we went to The Wetherby Whaler in Wakefield. After we went home and I used my PC. I cleaned my teeth.
19/08 Sun I used my PC until my driver/assistant called for me at 1200. We went to Tesco's to fill my car of diesel. We arrived at Odsal at 1300 and Tony the doorman greeted us and told we were stting at table 2. We had a roast Beef carvery followed a strawberry cheesecake. At 1455 we went outside to watch the match. Bradford were terrible and the half time score was 12 - 12 but the full time was 18 - 24. We went home and arrived at 1800. I ordered a 9" chilli pizza and chocolate fudge cake from Soprano's.

21/08 Tues Yesterday I had problems with my Catheter not draining. Early this morning the night nurses changed my Catheter but it didn't stop by passing. I asked the Bluebell nurse to take off my day bag, which the day carers had put on, my Catheter started draining. In the morning I used my PC. After lunch I used my PC. After dinner I cleaned my teeth and I used my PC.

After I got home 2215.

28/08 Tues I phoned Revitalise to book a holiday from Sat 17/11 to Sat 24/11 at Sandpipers Southport. They are sending the details. I used my PC to book a table at Capri's. After lunch I used my PC to change my passwords until 1800. I watched TV until Lee, my driver, came with his wife to take me to Capri in Horbury Bridge. We arrived at 1930, I ordered a Soda water, Minestrone soup, Chicken Cesare. It was served about 20 minutes after, my Minestrone soup used fresh vegetables, my Chicken Cesare was excellent. I got home at 2130.
29/08 Weds I got confirmation for my holiday with Revitalise. I took insurance with Age UK. I used my PC. After my lunch I used my PC. After tea I cleaned my teeth and used my PC.

My Life being Disabled June 2018
01/06 Fri I got ready to go to Pinderfields to get my Baclofen pump filled. My appointment was at 1345. I told the senior carer when I called at 0830 that I wanted my breakfast, two rashers of Bacon, 2 Sausages and 2 fried Eggs, at 1100. I said I'm ready for my Bladder wash from Thursday. She told me I shouldn't be calling at 0830 because the carers are having the handover. I told her I've an appointment at Pinderfiels at 1345 and my transport might at 1145. The carers came at 0915, they put me in front of my bathroom mirror where I cleaned my teeth and shave. I called again and they put me under shower where I had a shower. I called again they put on to my bed where they dried me and dressed me and put me in my electric wheelchair. The nurse washed my Bladder. At 1100 I went to the dining room but the cook hadn't made my breakfast ! I told him I wanted 2 rashers of Bacon, 2 Sausages and 2 fried Eggs. He cooked it and I ate it at 1200. I went my room and I used my PC. My transport came at 1230 and we(my carer and me) arrived at Pinderfields at 1330. I checked in and 1345 the nurse came and put me on the bed. The consultant filled my Baclofen pump and told me my next appointment will on 30/10. We went into the waiting room at 1415 and our transport arrived at 1515. We got home at 1615 and I used my PC to order a special Haddock fillet and a butter bread cake.

03/06 Sun I used my PC to ask Activities to take me to the TSB 119 The Headrow LS1 5HF After Sunday dinner I used my PC, put my feet up and watched TV. After tea I used my PC.
04/06 Mon I tried to phone TSB Leeds but I couldn't. I used my PC before lunch. After I used my PC until 1530 when Activities took me to the TSB in The Headrow, Leeds but couldn't find it until 1705 when it was closed. Activities told me they'll take me tomorrow. We got home at 1745 and after tea I used my PC.
05/06 Tues At 1130 Activities and a carer took me to TSB The Headrow, Leeds where I found out the fraud department had blocked my current account. I decided that the TSB is not for me ! I took all my money out and went home. After lunch I went to Tesco's to find out if their bank does current and saving accounts but they said use the internet. I went home, sorted my Morrison's shopping and had my tea. After tea I used my PC.
06/06 Weds I had 2 sausages, 2 rashers of bacon and 2 fried eggs for breakfast because I had an appointment with Wheelchair Services in Seacroft hospital at 1330 to get some new foot rests. I used my PC before my transport arrived at 1230. We got into Wheelchair Services at 1300 and went to get my new foot rests fitted by 1430. I got home at 1530 and went to the local Post Office to pay money into my Santander Account but they didn't know to do it ? I came back home and used my PC. After tea I used my PC.
07/06 Thurs I got up at 0700 because I had an appointment at Limb Salvage Clinic in Diabetic Medicine at 1130. My Patient Transport arrived at 1000 and arrived, my carer and me, at Manny Cussins Centre at 1030. We checked in and waited until 1130 when the nurse called me but she said you won't fit in the room with your wheelchair. She allocated another bigger room that somebody was using. She called again at 1145 and I went to another room. After 5 minutes the Head Podiatrist entered and tested my pulse in my ankles, feet and legs. He called a woman Dr she examined my notes and they called another person to tell me that my sores are nothing to do with Diabetes. The Podiatrist dressed my sores. While this was going on my carer got a text telling her her granddad has got worse. He's a patient in St James and she arranged for another carer to take over from her. He came when we had finished at 1315. The Activities driver had brought him and had waited in the car park. We got home at 1345 and I asked for 2 rashers of Bacon, 2 Sausages and 2 fried Eggs. After I went to my room knowing that younger brother was coming at 1500. He stayed until 1630 and I used my PC. After tea I cleaned my teeth and used my PC.
08/06 Fri I went to the Breakfast Club this morning. After lunch I spent all afternoon telling BT Pensions and the department of work and pensions that I'd changed my bank account. After tea I used my PC.
09/06 Sat I got ready to go to Odsal to watch Bradford v North Wales. The Inter Serve Health Care employee came for me at 1200. We arrived at Odsal at 1300, my driver/carer drop me off outside the Southbank Centre while he parked my car. We went inside and the doorman told us we were seating at table 2. The meal was Honey Roast Gammon Carvery, roast new potatoes and seasonal vegetables. Chocolate Fudge cake served with Chantilly cream. After our meal the MC gave us the team news and we had a quiz. 12/06 Tues The painters are decorating my room. I'm using Activities room. After lunch the heads of the departments held a meeting. I used Foxglove quiet room. It didn't finish until 1600 and painters had left my room a mess.in. I told the home manager and he got a carer to help him to sort my room. After I changed because I was going to Franco's Thorpe Park with my new driver/assistant and his wife. They came at 1850 and arrived at 1915. I had Chicken Milanese it was very good. After Lee took me home.
13/06 Weds I used my PC before lunch. After the acting Home Manager took me to Tesco's bank where I banked all my money that I withdrew from the TSB. After tea I used my PC until my eldest son came to attend the Relatives and Carer meeting. It was held in the Activities room. It lasted until 1930. I went to my room and used my PC.
14/06 Thurs I used my PC in the morning. After I used my PC but I got stuck twice in my wheelchair moving my electrical items. The second time the Home Manager came and plugged everything but 2 plugs were damaged. After tea I used my PC.
15/06 Fri I complained because I missed Breakfast Club because my carer took me until 1145 to cut my hair, clean my teeth, shave, shower, dry hoist me on to my bed, get dressed, hoist me into my wheelchair, go to the nurses station, get my blood/sugar, get my Insulin and go to the dining room. It was 1145 and had my breakfast. After I used my PC. After lunch I used my PC until 1500 when the inspector from CQC came to interview me. After I got changed because my sons were taking me out for an Indian meal. I used my PC.

We went in to Southbank Centre, Trevor Foster Suite and found our table. We had a drink and at 1745 we ordered our meal. Chicken Masala, rice and naan bread. After our meal we moved to the TV inside which was a 55". There was a 45m square screen on the pitch that involved going into the Provident stand. The match started and the half time score was 1 - 1. England won 1 -2 and we went home. I arranged that my driver/assistant take to the Cinnamon Lounge tomorrow. He's going to pick me up at 1830.

05/06 Tues At 1130 Activities and a carer took me to TSB The Headrow, Leeds where I found out the fraud department had blocked my current account. I decided that the TSB is not for me ! I took all my money out and went home. After lunch I went to Tesco's to find out if their bank does current and saving accounts but they said use the internet. I went home, sorted my Morrison's shopping and had my tea. After tea I used my PC.
06/06 Weds I had 2 sausages, 2 rashers of bacon and 2 fried eggs for breakfast because I had an appointment with Wheelchair Services in Seacroft hospital at 1330 to get some new foot rests. I used my PC before my transport arrived at 1230. We got into Wheelchair Services at 1300 and went to get my new foot rests fitted by 1430. I got home at 1530 and went to the local Post Office to pay money into my Santander Account but they didn't know to do it ? I came back home and used my PC. After tea I used my PC.
07/06 Thurs I got up at 0700 because I had an appointment at Limb Salvage Clinic in Diabetic Medicine at 1130. My Patient Transport arrived at 1000 and arrived, my carer and me, at Manny Cussins Centre at 1030. We checked in and waited until 1130 when the nurse called me but she said you won't fit in the room with your wheelchair. She allocated another bigger room that somebody was using. She called again at 1145 and I went to another room. After 5 minutes the Head Podiatrist entered and tested my pulse in my ankles, feet and legs. He called a woman Dr she examined my notes and they called another person to tell me that my sores are nothing to do with Diabetes. The Podiatrist dressed my sores. While this was going on my carer got a text telling her her granddad has got worse. He's a patient in St James and she arranged for another carer to take over from her. He came when we had finished at 1315. The Activities driver had brought him and had waited in the car park. We got home at 1345 and I asked for 2 rashers of Bacon, 2 Sausages and 2 fried Eggs. After I went to my room knowing that younger brother was coming at 1500. He stayed until 1630 and I used my PC. After tea I cleaned my teeth and used my PC.

We arrived at Odsal at 1300, my driver/carer drop me off outside the Southbank Centre while he parked my car. We went inside and the doorman told us we were seating at table 2. The meal was Honey Roast Gammon Carvery, roast new potatoes and seasonal vegetables. Chocolate Fudge cake served with Chantilly cream. After our meal the MC gave us the team news and we had a quiz. My Life being Disabled July 2018
01/07 Sun My nurse changed my Catheter this morning because it was bi-passing. After breakfast I used both PC's to change my passwords. After Sunday dinner I used my PC, put my feet up and watched TV. After tea I used my PC's and cleaned my teeth.
02/07 Mon I used my PC until lunch. After I used both PC's. After tea I used both PC's.
03/07 Tues I ordered 2 Hospitality tickets from Hunslet. I phoned Ross Wheelchair repair because my changer isn't working. They're coming tomorrow. I used my PC's before lunch. After I posted a photocopy of my passport, a photocopy of my entitlements and a passport size photo.I used my PC's after tea.
04/07 Weds My wheelchair is on 2 reds and 1151 Wheelchair maintenance aren't here ! I checked our handyman and he's got it. I put my electric wheelchair on charge. After lunch I used my PC and put my electric wheelchair on charge After I cleaned teeth and used my PC.
05/07 Thurs The OT who's in charge of the Activities told me to bill Ravensdale because I'm getting my driver to take me to Hunslet on 15/07. After lunch I used my PC. After tea I cleaned my teeth and used my PC's
06/07 Fri I used my PC and lunch, I bought a Haddock fillet and mushy peas through Just Eat. After I used both of my PC's but 1500 my Bowels opened. I told the carers who change my pad and put my shoes on. At 1600 my driver and his wife came to take me to Odsal to watch Bradford v Newcastle at 1945. We arrived at Odsal at 1700 and went into the Trevor Foster suite for a drink which we had outside watching the under 19's play. We went to the Legends suite for our meal at 1815. We had Steak and Ale pie, mustard mashed potatoes, carrots, garden peas and gravy. My pie was very large I couldn't eat it all. The dessert was Raspberry cheese cake, ice cream and cream. After we took our seats outside. The sun was shining from the west opposite the Southbank Centre. At half time the score was 14 - 4 and at full time the score was 24 - 4. The was over 3,000. We got home at 2245.

10/07 Tues This morning at 0830 I called for the nurse for my bladder wash. When she had finished I asked her to ask the carers if I could get up. The carers told me I was having a bed wash and I said I want a shower to which responded if your having shower you'll have wait until 1000. At 1000 I called the carers to take me into my bathroom to clean my teeth and shave. After I called the carers to put me under my shower and put shampoo in my hair plus wash my left arm and hand. I washed every thing else and rinse me. After I called my carers to me into my room, hoist me into my bed and dry me. They dressed me, hoisted me into my electric wheelchair and left me. I tidied my table and went to the dining room where I had breakfast. After I went to my room and use my PC. After lunch I went to my room until 1350 when I went to find out where I was interviewing. It was in quiet lounge with a carer that already works here. At 1430 I went upstairs to the Activities room to attend the Food Forum, After I went to my room and used my PCs. After tea I used my PCs.

12/07 Thurs I used my PC before lunch. After I went into the garden to read my book and sunbathe. After I felt sick and told my nurse. She phoned 999 for an ambulance because my temperate was really high(40') but they didn't come. At 1945 she phoned again and they took me to St James A & E at 2015. Later they transferred me to ICU and about 0300 they transferred me to ward J26 bed 14. I stayed there until Tuesday 17/07 when I was discharged. The NHS paid for a taxi and I got home at 1900.

24/07 Tues I visited Lakeview nursing home in Leigh that just been built. Sophia Feurtado the Service Users Engagement manager said I could. After we, my driver and me went to the Parsonage for lunch. I got home at 1600 and used my PC. After tea I used my PC.

27/07 Fri I used my PC and at 1100 I went to the Service Users breakfast club. After I put my feet up and watched TV until 1800 my Bowels opened. I told my carers who changed pad and shirt. At 1900 my youngest son came to me to take me to the Fox and Grapes for a meal. Is was rubbish.

My Life being Disabled May 2018
01/05 Tues I got up at 0700 because I was interviewing for the cooks job. I used my PC and at 0950 I went to the home managers office. At 1015 we decided that she wasn't coming. I went to my room to use my PC and at 1100 the Incontinence nurse came because this home had request to review my pads. She asked me to try another make. After lunch I went to Tesco's to get some bottles of water and a packet of crisps. When I got home I went to the Activities room to see Julie about this Sunday she's taking me to Odsal and on 22/05 she's taking me to the Service Users Council but she was out. When I was out I got an email from Tracey Bradford Bulls telling me The Masquerad------------------e Ball has been postponed and new date will be published later. I went to the Activities room to tell Julie about the Service Users Council that I've booked a table after the council but she wasn't there. After tea I used my PC.
02/05 Weds I used my PC before lunch. After I used my PC to change my passwords. I tried to buy from Tesco's a TV Sharp 55" LC-55CUG8052K SMART 4k U HD TV with Freeview HD for £399 and a Laptop HP17-BSO24NA but failed. After tea I used my PC.
03/05 Thurs I used PC and got letter from DWP about my DLA being replaced by Personal Independent Panel. I phoned PIP and answered few questions. They said an Investigator would contact me. After lunch I used my PC, put my feet up and watched TV. After tea I cleaned my teeth and used my PC.
04/05 Fri I went to the Breakfast Club in the Activities room at 1100. After I used my PC. After lunch I used my PC, put my feet up and a movie on my PC. After tea I cleaned my teeth and used my PC.

06/05 Sun I got ready to go to Odsal to watched Bradford v West Wales. We(Activities and me) set off at 1200. We arrived at 1240 and Julie drop me off at Southbank Centre. She parked my car in Disabled spot no. 51 and joined me at the entrance to the Southbank Centre. The doorman told us we were sitting on Table 2. I gave Julie money for drinks. The main was Roast Beef Carvery, Yorkshire pudding, roast potatoes and seasonal vegetables After the quiz we had Lemon Posset with Raspberries and Shortbread finger. The match started at 1500 it was all one sides because West Wales were poor. The half time score was 60 - 0 and the full time score was 124 - 0. We got home and I ordered a 9" Chilli pizza with a Chocolate Fudge cake.
07/05 Mon I got my new Laptop and I used old PC before lunch. After I used my new PC but at 1415 my bowels opened but there wasn't any carers to clean and change my pad. The senior carer told me it would be 30 minutes before a carer is available. The carers came into my room at 1500 but they didn't deal with me because my air filled mattress had been deflated. They restored it and told it will another 30 minutes. They came back to me at 1545 and cleaned me and changed my pad. After I used my new PC. After tea I used my new PC.
08/05 Tues I used my new HP PC. After lunch I went in the garden to read and sunbathe. After tea I used my PC.
09/05 Weds I used my PC. I told the cook I wanted my lunch at 1200. After lunch I used my PC expecting the taxi to arrive at 1300. When it didn't arrive at 1310 I went to the nurses station to inquire what's happening. The senior nurse told me there are no taxis available. She's phone Wheelchair Service to book another appointment. I went to my room, put my feet, watch my TV and a cup of coffee. At 1630 I ordered 1 Haddock fillet, 1 large mushy peas and 1 small mushy peas from Just Eat .After tea I used my PC and cleaned my teeth.
10/05 Thurs Our cook didn't attend today. The carers decided to take us for a pub lunch at 1300. We walked to the Pendas Arms which is 15 minutes away. I had a Soda water, All day breakfast and Chocolate Fudge cake. When I got home it was 1530 and used my PC. After tea I cleaned my teeth and used my PC.
11/05 Fri I used my PC because the Friday morning Coffee club had been put back to 1100. At 1050 a carer told me the Breakfast club had started. I went up in the lift to the Activities room and had 2 slices of toasted, a glass of orange juice and cup of coffee. At 1200 I went back to my room and used my PC. I went to Tesco's to buy some Sun tan lotion, water, Jakeman's and a Tesco's bag. When I got home I used my PC. After tea I used my PC.
12/05 Sat I was using my PC when at 1135 s carer told me Paul from ISHC my driver/carer is here. I got ready and went to see the nurse to get medicine. After I meet Paul in the reception. He driving to Odsal, Bradford. We arrived at 1245 in the doorman told us we're sitting on table 2. Our meal was served at 1330 and there was Char grilled Chicken, with Jacket potatoes wedges, green salad and coleslaw. Followed by Raspberry and White chocolate cheesecake served with Fruit Coulis and Chantilly cream. The match started at 1500 and at half time it was 30 - 6 and the full time it was 54 - 16. We got home at 1740 and I ordered a half pounder Chilli burger with onions and chips plus a jam rolly polly with custard.
13/05 Sun I used both PC's. After Sunday dinner I went to Tesco's to find a mini scart lead but I could find 1. I did some shopping and got at 1545. I used my PC and bought a mini scart to scart lead on e-bay. At 1700 I went to the TV lounge because it was another Service User's birthday. I had pizza and cake. I went to my room and used my PC.
14/05 I used my PC in the morning. After lunch I went to Tesco's to buy AAA batteries, 6 packets of Jakemans, Halls Soothers, Fishermans Friends, Crispz,. When I got home I used my PC. After tea I used my PC.
15/05 I used my PC in the morning and after lunch. At 1440 I went in the garden to sunbathe and read my book. At 1630 I used my PC. After tea I cleaned my teeth and used my PC.
16/05 I spent all morning filling the form 'How your disability affects you' that the DWP sent me. I used my PC. After lunch which was Lasagne(rubbish) I sent an email to Lynda complaining about it. After tea I used my PC.
17/05 I was woke at 0820 by the agency nurse taking my blood/sugar measurements. I asked her if she is washing my bladder but she didn't know anything about that. About 15 minutes later she returned to wash my bladder. At 0900 I told the carers I wanted to get up. After I went to the dining room to have breakfast but there wasn't any semi skimmed milk. After breakfast Activities had provided a singer because it was a Service User 40th birthday. That took me until 1200 when I went to my room and set up both PC's. I tried to get DVD to work with new Sharp TV. After lunch I emailed to the home manager with following Complaints, my wheelchair should be cleaned every Wednesday night, the Agency Nurse didn't knock on my door this morning, She took my blood sugar measurements and didn't wash my bladder. She came back 10 minutes to wash my bladder out, ther was no semi skimmed milk and lunch wasn't Corned Beef Hash. The handy man tried to get DVD working but he failed. I used my PC's updating my Complaints database. After tea I used my PC's
18/05 Fri I went to Breakfast club this morning. It was 'Italian' breakfast. After I went to my room and used my PC. After lunch, which was terrible, I used my PC. At 1500 I decided to go to Tesco's where I bought a DVD with HDMI, a HDMI lead and 2 packets of Polo's. After tea I used my PC.
19/05 Sat Everybody except for me is excited because of a Royal Wedding. I used my PC's in the morning. After lunch I used my PC, put my feet up and watch TV. After tea I cleaned my teeth and used my PC.
20/05 Sun I fitted my new DVD and used my PC before Sunday dinner. After I told the nurse my urine wasn't going in my bag. She gave me another bladder wash and change my bag. I put my feet up and watched TV. At 1630 I used my PC. After I cleaned my teeth and used PC.
21/05 I phoned the Bulls to transfer my tickets for The Masquerade to The Summer Ball. I had my lunch at 1200. Transport came for at 1315 and we arrived at St. James at 1400. I went to see the first person at 1415 she tested my eyes and put drops in. I went the second person after 15 minutes he looked into my eyes and everything O.K. Our transport came at 1530 and we got home at 1600. When I got into my room I used my PC and order a 9" Chilli pizza and a Chocolate Fudge Cake.
22/05 At 1100 I went to the Tankersley Manor Hotel because the Service User Council were having the bi-monthly meeting. It started at 1230 and finished at 1510. We got home at 1600 and logged at 1610, I ordered a Special Haddock Fillet and a Large Mushy Peas. They arrived at 1710 and after I cleaned teeth. I used my PC.
23/05 Weds I used my PC's before I met Wakefield Social Services at 1030. She decided to leave my Complaints with Ravensdale. I sent an email to my local MP Mary Creagh. My GP to look at my site because it was red and sore. She prescribe antibiotics. At lunch I was disturb by a person from Continuing Care. I told her to see me after lunch. At 1400 4 Huskies came but Continuing Care gave me a report. I went into the garden to met the Huskies. After I went to my room and use my PC. After I cleaned my teeth and used my PC.
24/05 Thurs I used my PC before lunch. After I used my PC, I went to Tesco's but I couldn't carry my shopping. When I got home I ordered the rest from Asda. After tea I used my PC.
25/05 Fri I went to the Spanish Breakfast Club this morning where I had a Spanish Omelette. After at 1145 I used my PC. After Fish, chips and mushy peas I sorted my Inter serve Healthcare Care Plan. The Senior Nurse came at 1500 and spent 60 minutes questioning me and she's going update my Care Plan. After I phoned TSB and found out somebodies been hacking into my account. The Fraudulent group have blocked my Internet and are looking into it,. After tea I cleaned my teeth and used my PC.
26/05 Sat I woke up at 0515 because my Catheter was blocked but the night nurse didn't know what to do? He sought assistance from the nurse up stair and she gave a Bladder wash. I woke again at 0630 because my Catheter was blocked again. The nurse from upstairs changed my Catheter. After breakfast I used my PC. My youngest son phoned to tell he is coming after lunch to take me out for a meal. I told the cook I only want a bacon and egg sandwich for lunch. After lunch he came to take me to Hawthorne Farm in Yeadon. I Farmers Beef Chilli and he had wrapped Yorkshire pudding. After he took me home at 1640. At 1700 I had Sirloin steak and after I used my PC.
27/05 Sun I used my PC in the morning. After Sunday dinner I went in the garden to find an electrical socket but there wasn't one. I wanted to listen to Hemel v Bradford which is kicking off at 1400. I went to my room to watch it on Facebook under Proper Sport. At first the broadcast was terrible but after 10 minutes it was OK. Bradford won 0 - 68.After I used my PC. After tea I booked a table for 5, an electric wheelchair(adult), 3 adults and a small child for tomorrow at 1930 for Mumtaz.
28/05 Bank Holiday Mon I used my PC and got my Care Plan to read. After lunch I used my PC. My eldest son phoned to tell me his fried and family aren't coming because their boy is sick. After lunch I used my PC. I didn't my tea because I was going out a meal with my oldest son. He picked up at 1900 and we went to Mumtaz but it was closed. We spent 1.5 hours looking a restaurant but there were none because it was Bank Holiday. When I got home I ordered a Chicken Garlic Chilli Masala and a Chocolate Fudge cake.
29/05 Tues This after I've an appointment to fill my Baclofin pump up at 1515. I used my PC and I got a letter the NHS Continuing Care which involved a questionnaire. It took me all morning and 1200 I went for my lunch. After I used my PC and waited for my transport to arrive. At 1545 Patient Transport phoned to tell her that they aren't picking me up. I emailed a complaint that never turned up. My nurse phoned Pinderfields and arranged another appointment on 13/06 at 1445. I started changing my passwords. After tea I used my PC.
30/05 Weds I reset my HP PC because it wasn't very well. After I sent an email to the CEO of Exemplar stating the worsening of Ravensdale. After lunch I used both of my PC's. After tea I used my PC.
31/05 Thurs My carers examined my feet because both feet had cuts on them. The senior carer told me Lynda the temporary House Manager want's to see me today. I use my PC's before lunch. After I used my PC and at 1530 I met with the House manager, the Clinical manager and the administer. After tea I used my PC.

My Life being Disabled April 2018
01/04 Sun I used my PC in the morning. After Sunday dinner I used my PC, put my feet and watched TV. After tea I used my PC.
02/04 Mon I got ready to watch Hunslet v Bradford in the Challenge Cup at the South Leeds stadium. My eldest came for me at 1120 and arrived at 1200. We went up 1 floor by an old lift and got seats near the exit. We had Vegetable soup and for main course Pork and Gammon with new potatoes and a variety of vegetables with gravy. After (the service was slow) I wanted to empty my bag. We went to the Disabled toilet which was in a small corridor. I just fit my wheelchair through the door. We emptied bag and I tried to reverse my wheelchair through the door and into the corridor. It was nearly kick off and the corridor was filling up with spectators who couldn't get to their seats. I backed up into the Disabled toilet to wait for the spectators to get through. I did a 15 point turn and eventually got out. We went to the Disabled viewing platform to watch the match. At half time the score was 6 - 14 with a full time score of 10 - 34. We're playing Warrington at Halliwell Jones on Saturday at 1500. When got home I ordered a Chilli pizza and a Chocolate Fudge cake.

05/04 Thurs I used my PC to write an email to Karen Neal complaining again. After lunch I used my PC. The Activities told me they're having a Food Forum this afternoon. They'll come down before it starts. I went to the Food forum which lasted until 1600. After I used my PC.
06/04 Fri I went to the Breakfast club whose theme was Continental. After I used my PC. After my lunch I used my PC, put my feet up and watched TV. After tea I used my PC.

09//04 Mon I used my PC to sent an email to Doncaster Commercial enquiring about the match with Bradford on 29/04. After lunch I used my PC at 1430 my bowels opened, I told the carers who changed my pad. I tried on my new Tuxedo size 50 it fits. I used my PC After tea I used my PC.
10/04 Tues I used my PC. After lunch I used my PC to register my Nectar card. Activities were having a 60's afternoon which included a quiz. It finished at 1630 and I went to my room. I used my PC. After tea I used my PC and cleaned my teeth.
11/04 Weds I tidied my top drawer and used my PC. I had an appointment with Wheelchair Services at 1130. My Patient transport didn't come at 1105 Activities came to take me. We used my car and got to Wheelchair services at 1130. They called me in at 1140 they took part of wheelchair off because they said it was too small. They measured my footrests to make some better ones. They found a piece of guarding that was catching and removed it. I finished at 1230 and we got home 1300. After lunch I tried to phone Doncaster Commercial but it was RTNR. I spoke to the Commercial team who said we're on the list but the man takes the money will contact me later on. I spent the rest of the afternoon using my PC working my spread sheet called Complains. After tea I sent an email to the Operations and Home manager a summary.

13/04/ Fri I used my PC to write an email to Lynda Holden complaining about Friday's fish. After lunch I read my gmail from Patient Transport which told me not phone them if I are going to/from Seacroft hospital because they don't have the contact. I put my feet up and watched TV. At 1635 I ordered a Fillet of Haddock and a large Mushy Peas from Rockfords via Just Eat. It came at 1725, I got 2 fillets of Haddock and a large Mushy Peas. After I check my electrical plus on my back wall and I charged my trimmer. I used my PC.
14/04 Sat I used my PC to put a bet on the Grand National. After lunch I used my PC, put my feet up and watched TV. After tea I used my PC.
15/0 Sun I got ready to go watched Bradford v Coventry at 1400 followed by Bradford v Leeds Girls. I used my PC. When John came from Inter serve health care we set off at 1100 and arrived at Odsal at 1150. The doorman told us we were seated on table 1. lIt served at 1230, Roast Gammon, new potatoes, carrots and red cabbage. Followed by strawberry cheesecake. At 1400 the men's match kicked off and half time it was 26 - 6 and at full time it was 52 - 6. After the women played Bradford v Leeds we stay until half time as Leeds were winning. I got home at 1730 and order a 9" Chilli pizza and a chocolate fudge cake from Soprano's.
16/04 Mon I used my PC in the morning. After I used my PC, put my feet up and watched TV. After tea I used my PC.
17/04 Tues Activities were taking me Bridlington setting off at 1030. I had Josh as my carerplus Julie Activities and we arrived 1245. We went to Baynham's for fish and chips. After we walked down to Trinity Road and onto the front. We walked back, down to the harbour and we went my car. We set off at 1530 and arrived home 1725. After my tea I cleaned my teeth and used my PC.
18/04 Weds I met with Lynda the Home Manager we decided I'll send her my Complaints.ods file and she'll arrange meeting with Heads of Departments to resolve my Complaints. After I used my PC and open my mail. A letter from the NHS informing me I've an appointment at Seacroft Hospital with the Specialist Rehabilitation Centre to get new foot rest on my Wheelchair. My 4 weekly bill from Ravensdale and parcel with my new toothbrushes. After lunch I used my PC and at 1445 my Bowels opened. My carers changed my pad. I used my pad. After tea I used my PC.
19/04 Thurs I sorted my little trolley but I wet my little drawer. I dried everything my little drawer. I put every thing back except my electric razor and the items that were in my trolley. I used my PC at 1215. I got my postal vote and this in. I gave it to a carer to post it. I got a letter Vision Express advertising they new eye test. I went ins read a book from and sunbath. From 1400 to 1530. and I went to my room to used my PC After tea I used my PC.
20/04 I went to Activities Breakfast Club we had bacon or sausage sanies. I came back to my room and used my PC. After lunch I used my PC and at 1400 met with Nicky Watson to deal with my complaint about urine going my mouth. After the meeting I went in the garden to read and sunbathe. I ordered a large fillet of Haddock and large mushy peas. They came at 1745. After tea I used my PC.
21/04 At 1100 I went to Warrington and we arrived at 1245. We had been upgraded from the Martin Dawes lounge to the Platinum lounge in the main stand. We got our tickets and went up 2 floors to the Platinum lounge. I had White Onion Soup with croutons, roast Pork, potatoes and vegetables plus cheesecake. Followed by Cheese and biscuits with coffee. At 1450 we went to take our places outside of box that nobody was using. At first Bradford were holding up but after 10 minutes the floodgates opened. The half time score was 30 - 0 and full time score was 54 - 6. We got home at 1830.
22/04 Sun I used my PC in the morning send an email to the Lynda the home manager complaining about the carer called Patrick who banged my head while he was hoisting me from wheelchair to my bed. After Sunday dinner I used my PC, put my feet

23/04 Mon I used my PC to send an email to Lynda the home manager complaining about Patrick driving my wheelchair into my back. She asked Nicky Watson to met me at 1100 to discuss this. Wakefield Social Care are coming tomorrow to see me. After lunch I used my PC, put my feet up and watched a movie on my PC. After tea I used my PC

26/04 Thurs I used my PC. After lunch I went to Seacroft shopping centre and bought some vitamin C effervescent. When I came back I use my PC. After tea I used my PC.
27/04 Fri I got up at 0700 because I was on an interview board for a cook at 1000 but she didn't turn up. I used my PC to distribute the Agenda for the Service Users Council on 22/05 at Tankersley Manor Hotel. After lunch I used my PC. At 1400 I had an interview for the cook's job. After I used my PC. After tea I used my PC.
28/04 Sat I used my PC before brunch. After I used my PC, put my feet up and watched TV. After tea I used my PC.
29/04 Sun I got ready to go to Doncaster to Bradford with my eldest son is driving. I used my PC and he came at 1150. We set off at 1205 and we arrived at 1310. Reception told us we had been up graded to a box that we were sharing another couple. At 1400 the waitress brought out main meal which was pan fried Chicken, mashed potatoes, roast potatoes, carrots, peas and gravy. Our dessert was Chocolate |Brownie with Vanilla Ice cream. At 1450 we went the Disabled viewing platform to watch. At half time it was 6 - 18 and at full time it was 6 - 30. I came home at 1800 and ordered a Chilli pizza and a Chocolate Fudge Cake from Soprano.
30/04 Mon I used my PC to change my passwords. After lunch I used my PC all afternoon. At 1630 I ordered a Chicken Garlic Chilli Masala and Chocolate Fudge cake from Soprano's.
